Mission Statement: The Cornell Scott-Hill Health Center mission is to provide compassionate, quality, accessible and culturally appropriate healthcare that integrates medical, behavioral health, dental and prevention services to eliminate health disparities and improve the health status of our patients and our community at large. Vision: We provide a model of quality and cost-effective health care for all that benefits our community. We advocate with others for our model to be an essential component of any effective solution to health care reform for our state and nation. Partnering with our patients, community, allies and staff is vital to the success and expansion of the service, capacity building, and the advocacy results we produce.

Profile:

Jamesina E. “Jai” Henderson joined Cornell Scott-Hill Health Center in July 2009 as Chief Executive Officer following an extensive national search conducted by the Corporation’s Board of Directors. She came to the Center after leading a pioneering community health center committed to underserved persons in Los Angeles. Throughout her career Jai has been an insightful and agile leader, collaborator and consensus builder. She has the demonstrated ability to build strong management teams. She fosters a leadership culture of ownership, engagement and creativity while role-modeling a character and expectation of high values and shared accountability. At CS-HHC she directs the efforts of a legacy institution with a 43 year history of innovative patient care. She encourages new thinking, challenges assumptions and set the organization on a path to achieve national recognition as the best example of integrated care in a community healthcare setting. Her leadership resulted in the CEO of the National Council for Community Behavioral Healthcare, a behavioral healthcare industry leadership group, stating that CS-HHC “is the best example of integrated primary, dental and behavioral healthcare in the nation.” Working with the community is at the core of Jai’s skills and experience. In addition to serving as the CEO of the T.H.E. Clinic in L.A. she worked as a project manager for Rebuild L.A., an organization created to help rebuild the City in the wake of the 1992 riots. Her success led to her appointment by then Gov. Wilson as the Executive Director of the State Business Revitalization Center, a business assistance program. While Executive Director of that program, Jai was appointed by the L.A. Mayor to serve as commissioner of the city’s Building & Safety Commission. A native of Washington, D.C., Jai received her B.A. degree in sociology/psychology from Howard University and her M.B.A. from the Wharton School of Finance and Commerce at the University of Pennsylvania.

Program Description:

Cornell Scott-Hill Health Center staff provided almost 31,500 people with a continuum of medical and oral health services during calendar year 2011 regardless of their insurance status, race/ethnicity or income level. The following characteristics describe the majority of those we served: Medicaid enrollees (65%); uninsured persons (21%); African American (30%); Latino (32%); and incomes below the poverty level (60%).   Our services include pediatrics, internal medicine, obstetrics/gynecology and dental. Our on-site pharmacy offers reduced cost medications for uninsured patients who qualify. Specialty medical services include cardiology, dermatology, ear, nose and throat (ENT), eye clinic (ophthalmology), gastroenterology, mammography, neurology and podiatry. We provide both preventive and restorative oral health services to children and adults.

Program Description:

Behavioral health services include outpatient substance abuse and mental health services for people of all ages. Also available are early interventions for developmental delays in children ages birth to three.   Counseling services are offered in individual, group, family or couples format. Psychiatric evaluations and medication management are available at each location.   More intensive services to assist in recovery from substance abuse include inpatient detoxification, outpatient detoxification, methadone maintenance and other medication assisted therapies, partial hospital and intensive outpatient programs. All are provided within our continuum of services. All Cornell Scott-Hill Health Center staff work closely together. Our behavioral health clinicians work with primary care physicians and pediatricians to treat all healthcare needs under one roof.

Impact Summary from the Nonprofit

A private, non-profit 501(c)(3) community health center, the Cornell Scott-Hill Health Center is a Federally Qualified Health Center and is accredited by The Joint Commission for the provision of medical, dental and behavioral health services. The Health Center is governed by a volunteer Board of Directors. Over 50% of the Directors always are consumers who use our services for their care. Our laboratory is accredited by the prestigious Commission on Laboratory Accreditation of the College of American Pathologists. Business New Haven and New Haven Magazine named us a Healthcare Hero 2011 in the Institution of the Year category.    During calendar year 2011, our staff of 500 people provided compassionate, quality, accessible and culturally appropriate health, oral health, behavioral health and enabling services to 33,315 people who received 196,445 visits. Our 16 care sites which include five school-based health centers are located in New Haven, West Haven, Ansonia, Derby, Milford and Branford. Our services are offered in many languages.   As a Federally Qualified Health Center, we provide health care services at reduced fees for uninsured people who meet federally set income guidelines. Our Pharmacy participates in the Federal 340B drug program which allows reduced pricing for medications for our uninsured patients who qualify. Also, we participate in several pharmaceutical assistance programs to benefit our patients. Our pharmacy programs saved our patients a total of $823,146 last year.   Many of our patients are at high risk for significant chronic conditions such as diabetes, cardiovascular disease and asthma as a result of being a person of color (70% of our patients), uninsured (21%) and having an income below the poverty line (59%). We have instituted targeted interventions to ameliorate these and other conditions of concern. The quality of all of our services are routinely monitored by our Board of Directors, providers and our Quality Assurance Committee.
