Program: Community Case Management

Budget:
$5,000
Category:
Human Services
Population Served:
Military/Veterans

Program Description:

As a part of the "Community Blueprint" we have added Community Case Management to our list of programs and services. Our Community Blueprint members work diligently to help connect Veterans and their families to the appropriate resources in their time of need. Thanks to the national presence of Community Blueprint, we are able to help Veterans from all over the United States. For example: We helped a Veteran and his family get their infant daughter to Boston where her life-saving surgery was preformed pro bono. We were also able to get their flight and stay compensated through various programs and services from other organizations. The idea of the Community Blueprint is to fill the gaps in services by connecting all organizations to one another thus creating a support system through which no one can fall.

Impact Summary from the Nonprofit

Since 2007, SSV has served over 8,000 Veterans and secured over $77 million in new benefits, services, and salaries for our Veteran clients. In 2012 SSV conducted almost 3,400 one-on-one counseling sessions and helped over 160 Veterans secure meaningful new careers with an average annual salary of $40,000. With our current growth rate and increasing partnerships throughout the state we hope to raise our numbers: we aim to place over 200 Veterans in new careers and conduct over 5,000 counseling sessions with our Veterans and their families. SSV's Keys to Success: Caring, knowledgeable case managers doing personal individual assessments and follow up Proactive counseling, guiding, and coaching Bridging the gaps/connecting the dots between Veterans and existing support services Orchestrating and leveraging already existing Federal and State support Optimizing communications with other agencies (VA, TRICARE, OSD, DOL) Facilitating good decisions, stress reduction and counseling Veterans on what to expect Orchestrating the entire process more efficiently
