Long Term Care & RehabilitationMercy Home:The Mercy Home is committed to providing quality care and a comfortable atmosphere to the residents to ensure their safety, health and well-being. With multiple levels of care, the Mercy Home provides services that cater to a wide range of individuals.The Mercy Home is located in Eagle Lake. It has 40 beds that are dually certified for long term care (nursing home), and skilled rehabilitation. Mercy Home also has 10 beds certified for residential care. We serve a diverse group of residents, from geriatric to younger individuals in need of nursing, rehabilitative, and residential services.

Home & Community Supports for Adults:Northern Maine General's Day Program provides facility and community based supports to individuals 18 years of age and older with intellectual and developmental disabilities, autism, and/or other related conditions. The purpose is to assist the individual to develop the skills necessary to reach their full potential for independent living and navigating in their community.Support can range from a few hours each week to 25 hours a week and provide treatment geared to assist a person to increase or re-learn independent living skills such as cooking and light housekeeping, physical and occupational fitness, personal hygiene, general and community safety, money management, accessing and engaging with community resources, and facilitating friendships and natural supports. The adult community support program is accredited by CARF.Qualified staff members are additionally trained in areas that include: person-centered planning, medication administration, positive behavioral supports and intervention, individual rights, first aid and CPR, environment safety, and diagnosis-specific to the person served.Home Support for Adults:NMG provides support to individuals with intellectual and physical disabilities, brain injuries, and other related conditions, in order to allow them to live an inclusive life within their communities. Qualified direct support professionals support individuals in their own homes or apartments or in an agency operated group home, otherwise known as waiver homes. Support can range from a few hours a week up to twenty-four hours per day and provides treatment geared to assist a person to increase or re-learn independent living skills, such as:- Money Management- Keeping Medical Appointments- Administering Medications- Facilitating Friendships and Natural Supports- Cooking and Light Housekeeping- Personal Hygiene- General Safety- Physical and Occupational FitnessHome supports are designed to serve specific target populations under the Maine Care Waiver programs:- Section 18: Adults with brain injury- Section 20: Adults with other related conditions - cerebral palsy or epilepsy or any other condition- Section 21 and 29: Adults with intellectual and physical disabilities and/or autismAdministering Agency for Shared Living Providers:Shared Living provides a home and supports for an adult with intellectual disabilities in the home of a qualified independent contractor. Shared Living is similar to foster care, but with the emphasis that the home is a cooperative sharing of space and supports between adults. In essence, the individual becomes part of the Shared Living Home Provider's life and family. The Agency role is to provide essential consultative services, such as:- Recruiting providers and assisting to match individuals with the appropriate home- Billing and disbursement of Maine Care dollars- Assessing for the individual and their team- Providing assurance on the person's quality of life and of the SLP's compliance of the Maine Care regulations.

Behavioral Health Services Outpatient Mental Health Counseling:Therapy/counseling for adults, children, and families. Disorders we treat:- Anxiety/stress management- Attention/hyperactivity disorders- Depression/bipolar/mood disorders- Eating disorders- Grief/loss- Personality disorders- Post traumatic stress & trauma- Psychotic disordersOptions for outpatient treatment:- Individual, family, & couples therapy- Home & community based treatment (HCT)Community Integration:NMG's Community Integration Services (CIS) program is a home and community based services designed to assist adults who have mental illness in Aroostook County. This program offers an array of services which includes, but is not limited to, the following: screening and assessment, individualized service planning, service linkage and coordination, service monitoring, and assisting with accessing funding for services.Skills Development:NMG's Skills Development program is a teaching based service designed to assist adults with mental illness who live in Aroostook County to learn skills necessary to improve their level of independent functioning in their home and community. Individuals are taught how to access community resources; increase natural supports; participate in educational, vocational, and social activities; effectively manage stress; resolve conflicts; solve problems; increase self-advocacy; and understand and self-administer medications.Daily Living Supports:NMG's Daily Living Support Services (DLS) program is a home based service designed to assist adults with mental illness who live in Aroostook County to maintain the highest level of independence possible. This service is provided in an individual's home and assists them with learning to cook and clean, shopping, organizing tasks, taking medications regularly, and maintaining safety in their home environment.Rehabilitative & Community Support Services for Children with Cognitive Impairments and Functional Limitations:The Children's Rehabilitiative & Community Support (CRCS) Program (Section 28) provides in-home and community services to children ages 20 or younger with developmental disabilities, functional impairments, and any diagnosis under the Autism spectrum. We are dedicated to assist our clients to reach their fullest potential, learn to express themselves with positive communication, and build healthy relationships. We encourage families to better understand their child's special needs. Staff are certified, skilled and proficient.Children's Home & Community Treatment Services (HCT)Children's Home and Community Treatment Services (HCT) is provided to children ages 20 years and under who have a diagnosis of mental illness, intellectual disability, autism or a pervasive developmental disorder. The goal of this service is to increase a child's level of social, emotional, and family functioning by enhancing mental and physical skill development.Case Management: Developmental Disabilities and Mental Health Diagnosis:NMG provides case management services to families who reside in Aroostook County. Case management is offered to children ages birth to 20 who have been diagnosed with a mental illness, an intellectual disability, a pervasive developmental disorder, and/or autism.
