Our programs
What are the organization's current programs, how do they measure success, and who do the programs serve?
Hilltown Community Health Centers programs include medical, dental and mental health services. We also provide a limited pharmacy program and some lab services. Our outreach programs include tobacco treatment, health care access, dental education and screenings, parenting education as well as eye care and medical care for elders in their homes. We also have a chronic disease management program. Our services are available to everyone though we make a special effort to provide accessible services to the poor and uninsured.
