What we aim to solve

Our community served is largely: 1) low income - 95% live below 200% of the Federal Poverty Level, 2) underinsured - 63% are covered by Medicaid, 11% are uninsured, and 6% are covered by Medicare, and 3) immigrant and/or best served in another language - 62% of our patients identify as Hispanic/Latinx. Our patients also manifest challenging social determinants of health such as high unemployment; homelessness and housing insecurity; underperforming education systems; food insecurity and poor nutrition; lack of exercise opportunities, and many others. As a result of all the above factors, including co-morbidities such as depression, substance abuse, diabetes and hypertension, our community experiences health disparities.

Health and Wellness

SECHC’s multicultural, multilingual and highly trained staff of 180+ serves over 19,000 patients accounting for 80,000 annual visits with an operating budget of $16.5 million. Our main clinic, located just steps from the South End’s Blackstone and Franklin Squares at 1601 Washington Street, is a 48,000 square foot state-of-the-art facility. SECHC’s Dr. Gerald Hass Center on Shawmut Ave. is Boston’s first full-time elementary school-based health center offering medical, behavioral and dental services to the local community and students of the Blackstone Innovation Elementary School.

SECHC’s high quality services and programs include primary care and prenatal medicine, behavioral health, dental and eye care. SECHC’s Quality Improvement program has been recommended as a national model by HRSA. Our specialty care services include addiction medicine/office based addiction treatment, civil surgeon/immigration physicals, diabetes management, family planning, HIV/AIDS & HCV testing and care, midwifery, neurology, nutrition, OB/GYN, pediatric asthma/pulmonology, pediatric dermatology, podiatry, and sexual health/STIs. SECHC also offers a variety of innovative programs, including Pediatric, Adult and Senior Wellness; Baby Café; Healthy Mom, Healthy Baby; Cooking Classes; Homeless Care; Earned Income Tax Clinics; and Women, Infants and Children (WIC). Our behavioral health department convenes individual and family support group meetings throughout the year.

Extending our longtime commitments to the integration of medical care and mental health services, and the education and training of healthcare professionals, SECHC launched New England’s first joint residency in Family Medicine and Psychiatry with BMC in 2018.

Our primary goal is Improve health outcomes in our community

South End Community Health Center is organized as a patient centered medical home. As such, our highly trained staff of 180 functions as a cohesive team in partnership with the patient to improve patient health and wellness. Care and services are culturally and linguistically sensitive for all, regardless of ability to pay.

We follow the United States Department of Health and Human Services Healthy People 2020 goals and objectives, which can be found here: https://www.cdc.gov/dhdsp/hp2020.htm. Our health center's electronic health record system supports our continuous quality improvement program, which insures strategic alignment across our organization and achievement of these goals.

South End Community Health Center employs a highly trained multicultural staff of 180.

2. A robust quality/performance improvement program and plan insures that we focused on our goals and monitor progress towards achieving them.
