Program 1
Project tlc provide short-term case management services to newly-released HIV-positive inmates the goal is to link clients to medical care, medical entitlements and stable housing while also providing basic qualjty-of-life items such as food, clothing and transportation assistance provided service to approximately 150 unduplicated clients during the six month period of July and December 2012
Program 2
Prevention conduct CDC-approved behavioral interventions with HIV-positive individuals and individuals who are at risk for contracting and transmitting HIV the goal is to reduce the rates of primary and secondary HIV infections as measured by follow-up surveys gauging client attitudes and behaviors around high-risk activities work with active drug users to facilitate entry into drug treatment programs provide aftercare relapse prevention services provided service to approximately 500
Program 3
MEDICAL CASE MANAGEMENT IDENTIFY CLIENT'S MEDICAL AND PSYCHOSOCIAL NEEDS, EVALUATE CLIENT'S MEDICAL FUNCTIONING AND FACILITATE ACCESS TO MEDICAL AND PSYCHOSOCIAL SERVICES THE GOAL IS TO INCREASE MEDICAL COMPLIANCE, MEDICATION ADHERENCE AND MEDICAL OUTCOMES AS MEASURED BY IMPROVED CD4 COUNTS AND VIRAL LOADS PROVIDED SERVICE TO APPROXIMATELY 410 CLIENTS DURING THE SIX MONTH PERIOD OF JULY AND DECEMBER 2012 (Code ) (Expenses 161,242 including grants of ) (Revenue ) MEALS PROVIDED BREAKFAST
