What are the organization's current programs, how do they measure success, and who do the programs serve?

PowerUp and Bear Power

Improving eating and physical activity with elementary school children in our area. Our PowerUp work continues to broaden and deepen its impact. Since the inception of the program 8,400 children and families have attended 200-plus open gyms, open skates, and dance parties hosted by PowerUp. 44,000 students in 20-plus schools have taken part in the Try For 5 School Challenge – a fun way to get kids excited about trying for 5 fruits and veggies every day. 1,700 kids have taken part in PowerUp cooking classes in partnership with Cooks of Crocus Hill (Stillwater) and Farm Table (Amery) and taken part in PowerUp recipe contests. 10,000 children and families took part in PowerUp Week activities across the St. Croix Valley and western Wisconsin.

Our agency/group includes: Hospice, Homecare, and Palliative Care. Most often, our patients move between and within these areas of care as they reach the move through a serious chronic illness and/or the end of their lives. This special kind of comprehensive care is provided by an interdisciplinary team and emphasizes quality of life for the person, in addition to supportive help for the family, during the time of illness and the bereavement period. This interdisciplinary team of employees and 92 hospice volunteers, work in close association to provide the best possible individualized care to each patient and their family/friends. We believe that providing services to the patient, and follow-up support to the family for 12-18 months after the death, is a vital component of Lakeview Health’s goal to provide the highest quality of care to our patients and their families across the lifespan.
