Hope Haven serves people from across the state of Iowa and in Southwest Minnesota. Annually we serve over 1,000 children and adults locally. Internationally, we distribute refurbish wheelchairs to over 8,000 people annually. We have 9 wheelchair workshops, plus 80 inmates that work to build wheelchairs in Springfield, SD and at the South Dakota State Penitentiary in Sioux Falls.

Hope Haven is community committed to the special creation of God that is the human being; encouraging the realization of dreams, desires, and aspirations, valuing gifts, talents, and contributions, sharing accountability for individual and community growth.

To help those live in their homes and in the community independently, provide competitive work services for those with disabilities, and to assist individuals and families to embrace hope, wellness, and recovery. Hope Haven International (HHI) reaches beyond the borders of our nation by extending mercy to people with disabilities around the world. The mission of Hope Haven International is to assist persons with disabilities to reach their potential. We do this by developing opportunities to improve the economic position, social welfare and independence of people with disabilities throughout the world. Hope Haven works closely with relief and development organizations, mission groups and individuals in various countries to deliver wheelchairs to those in desperate need. . Hope Haven International

EMPLOYMENT SERVICES
Total Persons Served: 618
Facility Based Employment
Key Outcome: 28% of clients discharged from FBE services moved to supported or competitive community employment, surpassing our target of 20%
Key Stat: 347 people were served in FBE.

Employee Development Services
Key Outcome: 100% of persons receiving work adjustment services engaged in integrated community employment services within six weeks of start date.
Key Stat: 47% of those served have a mental illness diagnosis.

Day Habilitation
Key Outcome: Community activity involvement has continued to increase each year with 90% of all service days including “offsite” community activities for person’s served.
Key Stat: 47% of persons served are 39 years of age or younger

Integrated Community Employment
Key Outcome: 97% of individuals in individual site Integrated Community Employment improved or maintained their hourly wage.
Key Stat: There were 98 individual site placements representing 60 employers.

Job Placement Services
Key Outcome: 90 individuals obtained individual site integrated competitive employment.
Key Stat: People were placed with 72 different employers.

Employment Planning Services
Key Outcome: 84% of persons served completed services within 90 days
Key Stat: 26% of those served were 21 years of age or younger.


RESIDENTIAL AND LIVING SERVICES
Total Persons Served: 574
Residential-Based Supported Community Living
Key Outcome: 78% retention rate for employees working 20+ hours a week
Key Stat: 14 residents are between ages 15-17.

RCF/ID Group Homes
Key Outcome: 89% of the clients served are satisfied with their residential setting.
Key Stat: 100% of those served were funded by HCBS waiver.

Westview ICF/ID
Key Outcome: Westview continues to experience transition displacement from a flood in June. They continue to reside at the old Long Term Care Facility in Orange City awaiting the construction of their new homes. Staff have done a great job of helping the clients through all of these transitions.
Key Stat: 50% were between ages of 40-54.

Waiver Homes
Key Outcome: 100% of individuals started services within 60 days of referral.
Key Stat: 162 people were served.

Supported Community Living / Adult
Key Outcome: 81% of new enrollees began services within 60 days of acceptance.
Key Stat: 46% of persons serviced have a diagnosis of mental illness.

Supported Community Living / Children
Key Outcome: 76% of new enrollees began services within 60 days from date of acceptance.
Key Stat: 119 people were served.

Respite
Key Outcome: 95% of persons served are satisfied with services.
Key Stat: 178 people were served.

Minnesota Services
Key Outcome: 77% retention rate for employees working 20+ hours a week.
Key Stat: 25% of those served were between 55-65 years of age.

MENTAL HEALTH & RECOVERY SERVICES
Total Persons Served: 1,405

Adult Mental Health Support Services
Key Outcome: 98% of persons served were free
