Mission

Founded by a group of Community leaders including Dean S. Lesher, former publisher of the Contra Costa Times, and Katherine M. Grant, a social worker, the Foundation is committed to providing vital services to aging persons who are at risk because they are alone, isolated, pressed by financial problems, or are confused. The focus of DVFA is to prevent inappropriate institutionalization of elders by providing them the necessary support to maintain them in community.

What are the organization's current programs, how do they measure success, and who do the programs serve?

Program 1

Some 1975, the Diablo Valley Foundation for the Aging has served over 5,000 older adults in Contra Costa county and has continued to be a leading agency in senior services. We offer five major service components: 1. Geriatric Care Management 2. Respite Center 3. Fiduciary services and money management 4. Representative payee and bill payer. Geriatric care management includes 1) Face-to-face consultation 2) Comprehensive needs assessment completed by a geriatric social worker 3) Coordination and implementation of social services 4) Monitoring and Supervision of personal assistants 5) Regular visitations and check-ins with client 6) Advocacy on behalf of client, the family, social services, and health care providers. DVFA offers money management services consisting of financial planning, bill paying, reconciling bank accounts, coordinating payment of taxes, assistance in selling real estate, transfer of stocks, and other investment transactions. DVFA serves as a representative (under a durable power of attorney program) in the event of an emergency or decline in health. Executor, Trustee, and conservatorship services are also available. DVFA also runs a respite center. The DVFA Valley Oak Respite Center provides an interactive program for frail seniors, those afflicted with Alzheimer's, related dementias, and physical disabilities. Seniors have the opportunity to participate in meaningful activities which encourage independence and fellowship.
