The link between poverty and health outcomes is well recognized. Recent Health Affairs Health Policy Briefs on these trends have shown a strong connection between people’s income and their health. It is also widely known that poverty is not an equal opportunist: Minority populations generally experience deeper poverty and thus poorer health outcomes. This fact was underscored by the community health assessments conducted for Roanoke, Virginia, in 2012 and again in 2015. Poverty levels in Roanoke track slightly above that found elsewhere in Virginia and in the nation as a whole. In medically underserved neighborhoods, however, these rates can exceed 50 percent. These areas have a disparity in life expectancy as great as six years from the median and 14 years from the highest reported in the city. Over 30 years ago, CHIP was formed with the goal of improving the health of children born into poverty so that they stood a better chance of growing up to become educated, contributing adults.

Home Visiting Program

Child Health Investment Partnership (CHIP) of Roanoke Valley is an early childhood home visiting program. CHIP of Roanoke pairs pregnant moms, low-income children, birth to first-grade entry, and their guardians with a Community Health Nurse and Family Case Manager to help them access needed medical services, to assist with the management of chronic conditions and to provide developmental education, kindergarten preparation and regular child assessment and monitoring. These services work to help build parenting confidence and to provide support to families as they set and reach goals moving toward self-sufficiency.

We also offer supplemental case management services that target specific needs: Prenatal Case Management, Health Care Coordination, Family Strengthening, Medically Needy & NICU Case Management, Oral Health Support, Asthma Case Management and Mental Health Counseling.

CHIP of Roanoke Valley offers health-focused, home visiting services to low-income families and their children (birth to age 7), utilizing teams of registered nurses, family case managers and a licensed mental health counselor. At enrollment, needs specific to each family are identified which may begin with pregnancy education. Depending upon the individualized needs of each family, CHIP connects enrolled caretakers and their children with doctors, specialists, pediatric dentists and a counselor if needed.

CHIP ensures that appointments are scheduled and kept, enhancing the health of each child. CHIP home visitors review medical records and coordinate care with the doctors and dentists that enrolled children visit. Transportation is provided to appointments to enhance access to care.

The following services are provided free to CHIP enrolled families:
OVERALL PHYSICAL HEALTH MANAGEMENT
CHIP establishes a medical home for newly enrolled children, communicates with doctors and reviews medical records to ensure each child meets immunization schedules and developmental milestones
PRENATAL & POSTPARTUM CARE
CHIP nurses work with expectant mothers of all ages to promote optimal birth weight, decrease infant mortality and stop or reduce high risk behaviors including smoking
ORAL HEALTH MANAGEMENT
CHIP connects each enrolled child to a dental home and applies fluoride varnishes from 9 months to 36 months to optimize their oral health and hosts Dental Days to provide in-kind dental screenings.
ASTHMA CASE MANAGEMENT
CHIP nurses provide monthly home visits designed to complement the primary care provider or pulmonologist care an asthmatic child receives
MENTAL HEALTH COUNSELING
CHIP’s Licensed Clinical Social Worker provides play therapy for enrolled children and therapeutic counseling for caretakers of CHIP children in CHIP’s Counseling Center
HEALTH LITERACY AND EDUCATION
CHIP educates parents to increase health literacy and communicate more effectively with doctors
TRANSPORTATION
CHIP provides more than 600 transports annually to appointments for CHIP clients
FAMILY SELF-SUFFICIENCY
CHIP caregivers are encouraged to establish goals and linkages with community resources, further parental education levels and maintain their child's developmental and literacy achievement to promote school readiness

Strategic goals are assessed twice a year to ensure goals are being reached.

--Less than 2% of asthma case managed children had visits to the emergency room over the past year thereby saving an average total cost of $172,730 for emergency room visits for one year.
--83% of postpartum moms breastfed their babies for the first two months
--92% of infant enrolled in CHIP within 30 days of birth had six well child visits within the first 15 months of life.
--375 transports to and from doctors visits were provided, further reducing barriers to accessing care
--80.69% of children under two years enrolled were screened for lead versus 66% of the Virginia Medicaid population.
--HEDIS measures tracked for all children receiving health care coordination through CHIP. These include Prenatal Care, Postpartum Care, Annual Dental Visit, Lead Screening, Immunizations (Combo 3), Well child Visits 3,4,5,6; Well Child Visits 15 months in addition to ER visits, Urgent Care visits, and many other measures.
