HCRHC (Lifespan Health) continues to improve health outcomes in a rural, economically distressed area. Access to healthcare is impacted due to the insurance status of many of the residents (either uninsured or underinsured). Lifespan is working to improve its care through expanding into a new facility that is designed for the team-based delivery of modern healthcare. Additionally, there is a tremendous need in its community for dental services and Lifespan is working to fund a dental program that will meet the needs of the poor and uninsured within its service area.

Community Health Centers are funded to make sure that all area residents have access to health care and to improve the overall health of their communities through provision of screenings, services, and education.

Lifespan wants to continue to provide high quality healthcare with some of the highest performance in response to HEDIS measures and wellness care - recognized by payers and its federal funders as a high performing organization while expanding its services and retaining professionals to deliver the care.

Lifespan will continue to pursue quality programs such as the IHI Triple Aim, Patient Centered Medical Home recognition, and other programs which keep staff focused on the delivery of patient centered, high quality healthcare which will improve health outcomes in its service area.
