Kōkua Mau's vision is a community where: the people of Hawai‘i are treated with dignity, compassion and love throughout their lives.

To make that vision a reality, Kōkua Mau's mission is: to weave a lei of caregiving and support so that the people of Hawai‘i facing serious illness can live in the place of their choice, with relief of pain and suffering and according to their values, beliefs and traditions.

Notes from the nonprofit

Kōkua Mau is Hawaii's trusted resource for accurate information on advance care planning (ACP) as well as hospice and palliative care. (Kōkua Mau means 'Continuous Care' in Hawaiian.)

Collaborating since 1999, we are a network of organizations and individuals we bring to the forefront meaningful, thoughtful and sensitive conversations on improving care and support for those with serious illness. We are viewed as a thought leader working to ensure that every individual has the right to quality care throughout their lives, especially at the end of life. We advocate for quality palliative and hospice care within the medical and legislative communities in Hawaii.

Kokua Mau strives to help people who may be facing serious illness and their loved ones to understand decisions they may need to make, find good information and resources in our community, and get the right care in the right place at the right time. Our experiences confirm the value of planning ahead. Talking about these issues before they are needed can help to prevent crisis and support people get the best care possible.

Kokua Mau Outreach and Training

Train professional health care workers in palliative care and advance care planning. Network with institutional leaders to leverage resources and staff trainings.

Kokua Mau serves as Hawaii's lead agency for POLST. We provide information, education and training throughout the state. Additionally we provide consultation to various healthcare agencies and organizations on POLST implementation including hospitals, clinics, health plans, long term care, emergency medical services and other who or implementing POLST. Kokua Mau staffs that statewide Task Force. Kokua Mau successfully initiated and guided new POLST legislation in 2014 to expand POLST access to include APRN.

Kōkua Mau wants to get our community talking about wishes for care at the end of life. Our new Let’s Talk Story Program will go out to community organizations, churches and temples with our free, 2-hour interactive sessions facilitated by Kōkua Mau’s trained experts. We know these are not easy conversations but they are some of the most important that you will ever have.

We want to help individuals and families to understand decisions they may need to make and find resources and information in our community to talk about their wishes for care at the end of life.

Our goal is to create a community in which advance care planning conversations are the norm. We are fostering a movement to change social norms in Hawai'i regarding conversations and decisions around serious illness and end-of-life care, and to provide all individuals and communities with greater choice and control regarding their quality of life. We do not focus on the end of life, but rather the quality of life. Advance care planning can take place years before an illness or crisis occurs and is the first part of the continuum of planning for one's legacy wishes.

Our 'Let's Talk Story Program' with our Advance Care Coordinator and her team of well-trained speakers can talk about all aspects of advance care planning, including documenting wishes in an Advance Healthcare Directive. We go out into the community island-wide and provide free tailor-made trainings and workshops where people work, live, play and pray. We know that these are not easy conversations but they are some of the most important you will ever have. We meet people in their environment where they feel comfortable and safe to address such difficult and emotional questions. Our goal: A community where these conversations are the norm and people's wishes and values are discussed, documented and honored and they are well cared for until their last breath.

Our 'Let's Talk Story Program' with our Advance Care Coordinator and her team of 19 well-trained speakers can talk about all aspects of advance care planning, including documenting wishes in an Advance Healthcare Directive.

Learn how to talk with your loved ones about what matters most to you.
Share about the kind of care you want to receive with your doctor.
Help you feel more confident and ready for the future.

Two Sessions: Let's Talk Story

The Let's Talk Story program is set up as two interconnected talks. We will work with you to determine what works best for your particular audience. We strongly encourage you to host both sessions, since our experience teaches us that this has the biggest impact.
The First Session: Exploring your ideas and beliefs

An interactive session which explores how you like to make decisions, who should be involved and the type of medical care you do and don't want. We also cover basics of advance care planning and preparing you for the conversation with loved ones.
The Second Session: Implementation

A hands-on implementation of the lessons learned in Session 1. One or two weeks apart, participants will have time to absorb the information and think it over. In this second interactive session, all are encouraged to ask questions fill out an Advance Directive to appoint an agent and document wishes for care.
