Mission

The mission of Mid-State Health Center is to provide sound primary health care to the community accessible to all regardless of the ability to pay.

As a health center in a rural part of New Hampshire, Mid-State Health Center finds itself on the leading edge of innovative, high-quality, patient-centered primary care. This approach helps residents of the communities we serve lead healthier lives and stretch their health care dollars further.

Our approach values and encourages treatment of the whole patient. The medical home model that we employ has proven to improve health outcomes, reduce costs and improve patient, family, physician and staff satisfaction.

By definition as a healthcare provider regardless of ability to pay, Mid-State is at its core a civic institution. What is unique about Mid-State is that we are driven by a single mantra: we do what is right for the patient.

Mid-State's philosophy of doing what is right for the patient extends to doing what is right for our community. Our staff is committed to the care of the entire community – not just our patients. Mid-State's leaders actively engage the community in the leadership of its practice through the participation of the community leaders serving on our Board of Directors and support its larger community by encouraging its professional staff and employees to actively participate in many venues.

Primary Healthcare Services

For a small health center in a rural part of New Hampshire, Mid-State Health Center is on the leading edge of innovative, high-quality, patient-centered care. This approach helps residents of the communities we serve lead healthier lives and stretch their health care dollars further. We are considered to be one of the leading community health centers in the New England region, specifically New Hampshire, as evidenced by our Level 3 medical home recognition by the National Committee for Quality Assurance (NCQA). Our approach values and encourages treatment of the whole patient. The medical home model that we employ has proven to improve health outcomes, reduce costs and improve patient, family, physician and staff satisfaction.

As part of Mid-State's integrated model, we offer preventive and restorative dental services to the community.

At Mid-State Health Center, we know that substance use from alcohol, opiates, and other substances is a complex mental health condition, one that is often accompanied by other symptoms and conditions. The Rise Recovery Team is committed to helping anyone recover from substance use disorders and other coexisting mental health conditions like depression and anxiety. We offer evidence-based, federally recognized treatment and recovery options proven to provide the best opportunity for success. Programs include Medication Assisted Treatment, Intensive Outpatient Program, Recovery Support, and an Impaired Drive Care Management Program.
