Our programs
What are the organization's current programs, how do they measure success, and who do the programs serve?
A Department of Public Health certified, nonprofit voluntary agency, accredited by JCAHO, the association today has more than 200 staff members who work seven days a week, 365 days a year. Additional services include treating children with catastrophic illness, HIV/AIDS services, certified nurses in oncology, cardiac and rehabilitation nursing, diabetes education, enterostomal and intravenous therapy, phlebotomy, discharge planning, phototherapy, and many more services. Our future work will be in providing a more diverse and sophisticated array of home based care to all ages.
Healthcare at Home
Chronic Care Management Program, which includes Cardiac, Diabetes, Oncology, Respiratory, Falls
Prevention, Wound Care and Chronic Obstructive Pulmonary Disease. Also, Medication Reconciliation,
Patient Coaching, Case Management, Coordination of Community Resources, and Discharge Planning. Nurses
are certified in diabetes education, oncology and wound care. The phlebotomy and IV services are provided
during home visits. The Caring for Kids Program provides Pediatric Nursing, Early Maternity Discharge, SIDS
Counseling, and care of fragile children due to their disease processes. The Rehabilitation Therapy
Department offers Physical Therapy, Speech and Language Pathology, and Occupational Therapy Services for
pediatric and adult patients. Home Health Aide Services, Medical Social Services, Registered Dietician
services and interpreter services are available for BVNA patients. The BVNA has a Care Transition Program
staffed by a team of Care Transition Specialists.
BVNA also provides:: “Call Us First”; a falls prevention program called “It’s a Joint Effort”®; Diabetes
Management Program recognized by the American Diabetic Association (ADA); Wound team intervention
lead by a certified wound ostomy continence nurse (WOCN); and Telehealth whereby patients are able to
communicate with the Brockton Visiting Nurse Telehealth nurse who monitors their vital health information
and immediately schedules a nursing visit or contacts patients’ physicians if necessary. Also, to meet current
needs BVNA recently added a Behavioral Health program, a Chronic Obstructive Pulmonary Disease
program, an orthopedic program for patients having knee and/or hip replacements and a Palliative Care
program. In addition there is a private duty program to provide unskilled care to patients in their homes in
order to maintain them safely where they want to be.
