What we aim to solve
Community Health Center aims to address the longstanding issue of lack of access to high-quality care for all individuals, regardless of their socio-economic status, gender, ethnicity, sexual orientation, etc. Over 7,500 residents of Barnstable County are fully uninsured, and many more members of our community experience additional barriers to care, making it increasingly difficult to maintain their health. Some examples include lack of transportation, no access to translation services, inability to afford insurance, food insecurity, housing insecurity, and many more. In addition, residents of the Cape are faced with a shortage of primary care providers, due to some doctors retiring and others moving into concierge practice. Coupled with an increase in population due to second home ownership and increased remote work opportunities, the demand for care at Community Health Center continues to grow.
Our programs
What are the organization's current programs, how do they measure success, and who do the programs serve?
Integrated health care for all who need it
Community Health Center provides integrated health care to these individuals who may not be able to afford care otherwise. Behavioral health screening services are integrated with primary care for patients, and CHC offers cognitive therapy, ongoing monitoring and support and prescription medication. CHC is strongly focused on integrated, innovative approaches to care, transforming the health of the patients served and evaluating programs and services regularly to address community needs.
Community Health Center Dental Services
Great oral health goes hand-in-hand with great overall wellness.
Community Health Center of Cape Cod has a full service dental clinic and oral health program in Mashpee for children and adult patients.
Our dental providers and staff at the Health Center work closely with your other care teams to coordinate your dental needs with your medical and behavioral health care needs.
And our dental services are available to all of our patients and to the entire community.

Complete Pharmacy Services
We try to take the stress and worry out of getting your medications with our convenient, affordable onsite Pharmacy.
The Pharmacy at Community Health Center provides affordable prescriptions written by your CHC provider (medical, dental, behavioral health and walk-in).
And our pharmacists do more than just fill prescriptions — they are a valuable resource you can turn to in order to ensure you fully understand your medications, any possible harmful interactions, contraindications, meet current therapy evidence based guidelines and can find alternatives to reduce cost and the number of medications you are taking.
Our two onsite pharmacies in Mashpee and Bourne are a great convenience for patients, with the added comfort of knowing your provider and pharmacists work together as part of your healthcare team.

CHC Optometry
Complete Eye Care
Taking care of your eyes is an important component of your overall health.
Community Health Center of Cape Cod offers a comprehensive Optometry Clinic that provides extensive vision services — from routine eye exams to advanced testing for vision-related diseases to providing affordable eyeglasses and contact lenses.
Although many eye conditions are preventable, many people do not get recommended vision screenings and exams. Regular visits with an eye care professional for a comprehensive dilated eye exam can help to detect common vision problems and eye diseases, including diabetic retinopathy, glaucoma, cataract and age-related macular degeneration.
Uncorrected vision can lead to inability to drive, challenges in reading and playing sports, headaches and strained eyesight. The Health Center offers affordable optometry services to help patients improve their eye health and avoid these challenges that come with vision problems. Optometry services are currently available to Community Health Center of Cape Cod patients only.

What is the organization aiming to accomplish?
The goal of our organization is to improve the health of our community through the provision of comprehensive, high quality, compassionate health care to all, regardless of their ability to pay. The Health Center cares for patients from all Cape towns, Martha's Vineyard and towns across Southeastern Massachusetts. 52% of Health Center patients have MassHealth or another state insurance plan, 22% have Medicare coverage and 5% have no insurance. Health Center enrollment staff assisted more than 3,000 patients and members of the community with the insurance application last year. Community Health Center directly addresses these problems and additional barriers to care by providing immediate assistance to these individuals from expert staff, trained to assist in any situation by providing resources and guiding them through the process. We aim to be a health care home of choice reflecting community needs, empowering individuals towards lifelong wellness.
What are the organization's key strategies for making this happen?
Our strategic plan encompasses our organizational mission, vision and values. It is a compilation of needs assessed within our community and then thoughtful and inclusive input from multiple stakeholders, including consumers, fellow human services and health care providers, staff and directors. Reflective of our commitment to excellence, the directors and staff support this plan as a guide for our continuous improvement, programming, business development and innovation in the upcoming years.
What are the organization's capabilities for doing this?
Our organization has an excellent staff and all are required to undergo the STAR quality training program that holds the staff accountable for making the most efficient and best decisions possible in terms of care at the health center. This allows us to become the quality patient-centered medical home we aim to be. Furthermore, our providers work in teams to manage a patient's overall health, which is part of our complex care program. In this way, all of a patient's healthcare needs are addressed at one time, including medical, behavioral, dental and optical health.
What have they accomplished so far and what's next?
In 1998, Community Health Center was founded as the Falmouth Free Clinic, an all-volunteer clinic caring for uninsured adults. The organization has evolved since then in response to the community's need for affordable, high quality, comprehensive services and now cares for more than 26,000 patients. Community Health Center received designation in 2006 by the federal government as a Federally Qualified Health Center (FQHC), and in October 2013, was awarded Level 3 (the highest level) Recognition as a patient-centered medical home (PCMH) by the National Committee for Quality Assurance. As a complete medical home, CHC offers primary care, behavioral health, dental care, optometry, affordable pharmacy services, x-ray and ultrasound services, case management services, barrier assessments and assistance, veterans' care, women's health services, addiction recovery services, teen and adolescent care and many other specialized programs. The Health Center has facilities in Mashpee, Falmouth, Bourne, Sandwich, and Centerville. The staff of Community Health Center is currently made up of 218 employees, led by an experienced and stable management team. Community Health Center has served for 25 years as the health care safety net and provider of choice for all who need care, especially the most vulnerable. Community Health Center is committed to meeting the health care needs of the community by providing high-quality care in a cost-efficient manner.
