Provide care and support to individuals and families facing advanced illness (defined as those with a life expectancy of six months or less.

Programs
Hospice services are provided by an interdisciplinary team of highly skilled health care professionals, including: physicians, nurses, social workers, spiritual caregivers, bereavement counselors, volunteers and other specialists as needed. All are trained to work with individuals diagnosed with a serious illness. Bereavement support and guidance are also provided to the family members.

What is the organization aiming to accomplish?
Increase the number of individuals and families receiving hospice care.
Expand the number of underserved individuals who receive hospice care.
Expand our reach into underserved areas throughout Rockland and Orange counties in New York.
Provide a deeper understanding of Hospice care for health care providers and community members to assist with patient choice for end of life care.
What are the organization's key strategies for making this happen?
Hire and place hospital liaisons in area hospitals to increase awareness and understanding of Hospice services for those caring for the seriously ill.
Continue training opportunities for those working in healthcare.
Continue to expand our expert direct service staff to serve the needs of patients and families throughout our community.
What are the organization's capabilities for doing this?
We are a NYS licensed, Medicare certified, Joint Commission accredited hospice program with an operating certificate that allows us to serve both Rockland and Orange Counties. Our 34 years of experience in providing hospice care in our community provides us with the expertise and knowledge to continue to provide excellent care to those in need in our community for decades to come.
What have they accomplished so far and what's next?
We continue outreach throughout our community in health care and community settings. We advertise with frequency and have begun a new training series at local area hospitals. We have begun the expansion of our services to include a palliative care program to serve more patients. We have continued a partnership with local area institutions of higher education to provide nursing students the opportunity to learn about end of life care.
