Programs

Hospice services are provided by an interdisciplinary team of highly skilled health care professionals, including: physicians, nurses, social workers, spiritual caregivers, bereavement counselors, volunteers and other specialists as needed. All are trained to work with individuals diagnosed with a serious illness. Bereavement support and guidance are also provided to the family members.

Increase the number of individuals and families receiving hospice care
Expand the number of underserved individuals (present focus: Latino individuals) who receive hospice care
Expand our reach into Orange County as requested by health care provideres

Hire and place hospital liaisons in area hospitals to increase the number of hospital referrals received
Obtain grant funding to support our Latino outreach program
Hire a full Orange County team and open a satellite office in Orange ounty

We are a NYS licensed, Medicare certified, Joint Commission accredited hospice program with an operating certificate that allows us to serve both Rockland and Orange Counties. Our 32 years of experience in providing hospice care enables us to develop a strategic plan to implement our goals. We have the full support of our board of directors and adequate resources to move these initiatives forward.

Recruited and hired a Chief Operating Officer
Secured a one million dollar grant to support our Latino outreach program.
Have secured a realtor to aid us in identifying office space in Orange County.
Have successfully hired multiple bilingual and bicultural staff and will continue to do so till all positions are filled.
Have translated brochures, patient/family documents, etc. to Spanish
We are in the process of developing a Latino outreach marketing program.
