What we aim to solve
CenterPlace Health (CPH)is a federally qualified health center (FQHC) and the areas largest primary care safety net provider for low-income and uninsured children, adolescents, adults and seniors. Without the services of an FQHC, uninsured patients with limited means, who may or may not have Medicaid coverage, have few options to receive primary care. They may go to private physicians who accept Medicaid (limited) or hospital emergency departments. CPH serves over 27,000 patients per year from in and around Sarasota County as well as portions of Manatee and Charlotte Counties. CPHs primary target population is individuals in the Sarasota region whose family size and income places them at less than 200% of the Federal Poverty Guidelines (<$50,201 per year for a family of four). Pregnant women and children comprise the majority of patients who use its services.

Primary Health Care
CenterPlace Health is Sarasota County's safety-net health care provider, delivering primary care services to over 27,000 patients each year. For many in our community -- those who are uninsured or underinsured-- CenterPlace Health is the only option for quality, affordable care. Though many of our patients have private insurance, Medicaid or Medicare, no one is turned away because of their inability to pay. All are welcome!

On-Site Behavioral Health Services
In an effort to provide integrated primary care and behavioral health to our primary care patients, CenterPlace Health expanded Behavioral Health services and locations in 2023. The program now includes 1 psychologist, two psychiatrists and a psychiatric APRN as well as 3 licensed clinical social workers, 5 case managers and 1 social worker. This is in addition to a program director and patient service representative.
We also contract with numerous community partners to services for those with substance abuse problems.
A special program provides case management services for at risk individuals (I.e., homeless, incarcerated, referrals from county courts) to facilitate completion of treatment programs and connect the individuals with wrap-around services to help stabilize their recovery.
340B Contract Pharmacy at Three Sites
CenterPlace Health works with a contractor to dispense 340B and pharmaceuticals to our patients. For the convenience of patients, there are contract pharmacies at three of CenterPlace Heath Primary Care locations. The federal 340B program allows low-income individuals receiving services at CenterPlace Health access to needed medications at a fraction of their retail price.
