What we aim to solve

SOURCE: Self-reported by organization

CenterPlace Health (CPH)is a federally qualified health center (FQHC) and the areas largest primary care safety net provider for low-income and uninsured children, adolescents, adults and seniors. Without the services of an FQHC, uninsured patients with limited means, who may or may not have Medicaid coverage, have few options to receive primary care. They may go to private physicians who accept Medicaid (limited) or hospital emergency departments. CPH serves over 27,000 patients per year from in and around Sarasota County as well as portions of Manatee and Charlotte Counties. CPHs primary target population is individuals in the Sarasota region whose family size and income places them at less than 200% of the Federal Poverty Guidelines (<$50,201 per year for a family of four). Pregnant women and children comprise the majority of patients who use its services.
