Mission

Advance Housing Inc's, mission is to offer individuals an array of housing and wellness services to enhance community integration and promote quality of life. Advance Housing continues to expand the range and scope of suportive housing opportunities available to individuals who struggle with issues of mental illness, substance abuse and homelessness. Our array of outreach services and property management are designed to help people live independently and productively in the community, we strive to exceed the highest standards.

What are the organization's current programs, how do they measure success, and who do the programs serve?

Supportive Living Progam-Bergen and Sussex Counties

The Supportive Living Program (SLP) provides services to individuals in Bergen and Sussex Counties with a mental health diagnosis. Referrals to SLP come from group homes, state and county psychiatric hospitals, homeless shelters, and community referrals. Individuals reside in 1-2 bedroom apartments or shared houses and they hold their own lease. Support services are provided and include medication monitoring, assistance with medical appointments, assistance with food shopping, socialization and other services designed to optimize their independence in the community. On call services are available 24 hours a day 365 days a year.This Program Service includes columns in the budget with the heading: SLP B, SLP Sussex, SLP S McKinney, and Sussex-Lafayette. SLP S McKinney is a homeless program in Sussex County that uses a "Housing First" model. The infrastructure in Sussex does not support a separate program element for homeless programs so this is combined with the SLP Sussex Program. The SLP Bergen Program reserves 27 placements(beds) for referrals from DMHS. The SLP Sussex Program reserves 6 placements(beds) for referrals from DMHS. The average staff to consumer ratio for this program is 1 to10.

The ACCEP program provides services to 42 individuals in Bergen County  and 30 from Union County who are directly discharged from the State Psychiatric Hospitals. Individuals reside in their own apartments (1 bedroom) and can hold their own lease. Support services are flexible to indiviudal needs made possible by approximately a 1 to 6 consumer ratio. On call services are available 24 hours a day, 365 days a year.

The HoST program provides services to individuals in Bergen County who are chronically homeless according to HUD's definition, and, have a severe and persistent mental illness. Individuals reside in their own one bedroom apartments or live in shared housing. This program utilizes a "Housing First" model. Support services are flexible to indiviudal needs made possible by a 1 to 8 consumer ratio. On call services are available 24 hours a day 365 days a year.

Population(s) Served

The Aging Out Youth Program provides services to young adults between the ages of 18 and 21 with a mental health diagnosis. Referrals to this program include individuals who are at riskof institutionalization and/or homeless and come from Northern Region providers including Childrens Aid and Family Services (CAFS). Individuals reside in 1 or 2 bedroom apartments and they hold their own lease. Support services are provided and include medication monitoring, assistance with medical appointments, assistance with food shopping, socialization and other services designed to optimize their independence in the community. On call services are available 24 hours a day 365 days a year. The avaerage staff to consumer ratio for this program is 1 to 6.

The second compoenent to a permanent Supportive Housing Programs is good effective property management.  Advance Housing owns over 35 properties in Sussex and Bergen Counties.  These properties include multi family properties, shared living, and one and two bedroom condos.  All Advance Housing consumers have a lease either directly with Advance Housing, or with a landlord in a market rate apartment.  Property management Services acts as a compassionate landlord or in many cases, as a liaison to a Public Housing Authority, to a landlord, or to the clinical team.
