Medical Case Management
Medical Case Management (MCM) is a program for people living with HIV/AIDS that offers supportive, one-on-one case management services within a harm reduction framework. Our Medical Case Managers specialize in working with people who inject drugs, are unhoused, or face other challenges that make them especially vulnerable and in need of extra support managing their HIV care.
Case Managers establish long-term relationships with clients and work closely with them to coordinate medical and behavioral health care, navigate complex bureaucratic systems to access benefits and services, create individualized care plans, and support clients in meeting their goals. We also educate and empower clients to advocate for themselves and their needs in medical settings and in everyday life.
