In our community there are vulnerable, low income elderly and home bound patients and their caregivers, particularly from ethnic minorities, that cannot access high quality, holistic primary health care that would allow them to age in place. Empowerment of patients and shared-decision making is difficult when language is a barrier. At Care for your Health we speak English, Spanish and Portuguese.

High-quality primary health care for elderly home bound patients to support aging in place, decrease visits to the ER and preventable hospital admissions, to lower health care cost of complex patients.

We aim to provide high-quality primary health care and geriatric care in-office and at-home to under-served, vulnerable, low-income and under-insured elders and home bound patients, particularly from ethnic minorities.

We promote aging in place, support life changes and life enjoyment.

We put patients at the center of our care plan, whenever possible develop care plans together with the patient and when appropriate, also with the main family caregivers.

We provide palliative and end-of-life care.

Considering the changing needs of the aging population and the scarcity of domiciliary primary health care, the Board of Directors at C4YH decided to focus on this aspect of health care. It was determined that the initial expansion of this program would require funding from grants, which would be mainly used to increase the number of staff and thus the number of patients accessing our at-home health care delivery program.

We have a board-certified specialist in Geriatrics and Internal Medicine; a Care Coordinator; a Home Visit Coordinator; and a
Home Visit Provider Nurse Practitioner.

All our staff is highly experienced and motivated.

Recent Accomplishments: During 2018, the rate of advanced directives in the home visiting program increased from 39% to 61%. Additionally, twenty care plans for patients in the home visiting program were completed during 2018. Of the patients that died in 2018, 73% died either at home or in hospice. Only 27% of the patients died in hospital.

In 2018 we received Premium Status with Cigna and United. In addition, although our Patient Risk is almost triple that of our peers in the area (C4YH HCC 3.3, vs average peers HCC 1.2), our cost is under double. These numbers are not only important for our organization, but for the global cost of care of high risk elders in the community.
