What we aim to solve
HCHOTV offers a Medicare-certified home health and a private duty home health agency, palliative care services, and a Medicare-certified hospice, along with a community-wide grief and bereavement program. Our goal and mission continue to be focused on "caring for our community," and keeping families in our valleys instead of relocating as a result of inadequate community-based programs for care in one's home. Our 6,000+ square-mile service area is supported by our office in Glenwood Springs providing either home care or hospice services for residents of Pitkin, Garfield, Eagle, and small portions of Gunnison and Summit counties. HCHOTV is the only provider that accepts all insurances and accepts charity care patients.
Our programs

Home Health Services
Our Home Health Services provide a wide range of health care services that can be given in the homr for an illness or injury. Home health care is usually less expensive, more convenient, and just as effective as the care received in a hospital or skilled nursing facility.
Palliative Care Services
Palliative Care Services provide relief from the symptoms and stress of a life-threatening illness that has not yet advanced to the point of needing hospice care.
Hospice Services
Hospice Services provide care to individuals who are diagnosed with a life-limiting illness and have a prognosis of six months or less to live, as well as support to the patient's family and friends.
Private Pay Services
Private Pay Services offer private duty nursing by certifited nursing aides or personal care assistants. Care provided is a two-hour minimum to 24-hours/day, seven days per week
Grief and Bereavement Services
Grief and Bereavement Services are designed to respond specifically to the needs of individuals and families who are coping with death, loss and grief.

What is the organization aiming to accomplish?
Our mission is to provide compassionate and dignified palliative and end of life care to patients and their families. Our current goals are:
1. To become the major provider of home-based services for residents living in our service area.
2. To establish a comprehensive array of home-based services from short-term intermittent care to transitional (palliative) care to end of life care.
3. To establish an array of programs and services that meet the need of our youngest to the eldest member of any community served by HCHOTV. To be the leader in grief support services for both children and adults.
