What we aim to solve
In Michigan there are over 790,000 individuals without health insurance. Not having health insurance creates a barrier to getting needed medical care. FernCare Free Clinic, Inc. provides that needed care to people between the ages of 19 – 64 that do not have health insurance. FernCare offers primary care services, lab, pharmacy and assistance enrolling in qualifying health plans. FernCare offers these services at no cost to the patient.

Clinic programs
FernCare is a free primary care medical clinic open to anyone between the ages of 19 - 64 who has no health insurance or primary care provider. In addition to the clinic, we do the common lab tests, have a dispensing generic medication pharmacy, operate a Counseling program, have insurance specialists at every clinic session and a robust referring network for those issues that are beyond our Scope of Service. For the general community, we are part of the DEA Drug Take Back program as a drop off site for no longer used medications and run a comprehensive health resource center available by phone, e-mail or hard copy. We have only two part time employees, everyone else is a volunteer including the Medical Director.

What is the organization aiming to accomplish?
1.) Enhance the quality of life t our patients by providing free quality health care and related services to qualifying patients.
2.) Create and maintain a sustainable model for the delivery of the care.
3. Expand the scope and range of services offered to our patients.

What are the organization's key strategies for making this happen?
1.) Sustainable Clinic-
Continue to look at potential grantor sources. Current grantors have helped to ease some of the burden of fundraising. We are looking to obtain our first Independant Audit this year which opens up a larger pool of future sources.
2.) Expand Services offered-
Quarterly meetings held by the Clinic team, and some input from the board of directors helps to grow the ideas on how to offer more services with little or no greater cost. One way we recently did this was offering family counseling for more simple issues and are trying to coordinate a connection between metal health and the overall medical health of those patients.
3.) Community Services-
With the Affordable Care Act in place, we have been asked 'Are we still relevant'. The simple answer is yes. There are still many people who still cannot afford health insurance nor are they eligible for the newly expanded Healthy Michigan. However, we know there is more that we can do in our community and have been checking with our insurance to see what is allowed, as well as starting the process to get information from the community for what they would like to see.
What are the organization's capabilities for doing this?
FernCare relies on the talent and creativity of 4 part time employees and over 60 volunteers, both medical and administrative. FernCafre relies on partnerships to drive outcomes. This includes our volunteer base bujt also partnerships. FernCare is a member of the National Association of Free Clinics, Michigan Association of Free Clinics, various Chambers and business related resources. FernCare has a mixed revenue model drawing funds from grants, fund raisers and direct individual donations.
What have they accomplished so far and what's next?
What we've done so far...
FernCare was established in a permeant location in 2011, from previously serving patients from temporary sites. Since that times services have been expanded to 6 clinic sessions a month, providing over 1,000 patient visits annually. The range of services has been increased to include primary care, pharmacy, ab services and either direct or referred specialty care. FernCare also implemented the use of electronic medical records
What's Next:
In the next 12 months FernCare is going to expand to 8 clinic sessions a month. FernCare is also increasing its capacity by offering Telehealth Services. FernCare is also launching an integrated case management program for our patients with chronic disease, focusing initially on diabetes and hypertension.
