Improving Access to Well-Woman Care Services for At-Risk Women

Goal: Improve Access to Well-Women Care Services for At-Risk Women. Objective: At least 200 at-risk women per year will achieve improved health outcomes, as measured by BCHC's electronic health records system. Program Activities: a) Annual preventive clinical care services for women; b) Pap smear and mammograms screenings; c) Counseling for sexually transmitted infections; d) Counseling and screening for human immune-deficiency virus; e) Contraception education; f) Breastfeeding support, supplies and counseling; and g) Screenings and counseling for interpersonal and domestic violence.

Population(s) Served

Goal: Expanding Access to Full Prenatal Care for Underserved Women. Objective: At least 200 at-risk pregnant women per year will access first trimester prenatal care and optimize the health outcomes of pregnancy for mothers and infants, as measured by BCHC's electronic health records system. Activities: a) Provide free access to pregnancy tests and ultrasound services;b) Use clinical information systems to accurately identify prenatal patients;c) Provide access to prenatal screening (blood pressure, diabetes, STDs, etc) and services;d) Provide education regarding contraception, preconception, and preventative care;e) Provide education on signs of pregnancy and the importance of early prenatal care;f) Provide educational resources regarding the importance of early prenatal care;g) Provide education and access to free prenatal multivitamins;h) Provide initial and ongoing education for providers and staff regarding importance of timely prenatal care; and,i) Provide continuing prenatal care through the second and third trimester of pregnancy in our Orange Clinic with the immediate goal of expanding to our Mission Viejo Clinic.

Population(s) Served

Goal: Improve Early Identification and Treatment of Sexually Transmitted Diseases. Objective: Improve the identification and treatment of sexually transmitted diseases for at least 4,000 individuals per year, as measured by BCHC's electronic health records (Kareo). Program Activities: a) Provide routine screenings for common STDs (Gonorrhea, Chlamydia, Hepatitis B, Syphilis) for all sexually active patients;b) Provide STDs risk reduction and counseling education (abstinence);c) Provide STD treatment and patient follow up.

Population(s) Served

Obria Medical Clinics offers free access to evidence-based health education for low-income and under-served individuals, so they can adopt healthy lifestyles and achieve optimal health outcomes. The following classes and mentoring opportunities are offered as part of the Obria's comprehensive health education program: BeginnigsGoal: Improve child development and healthy pregnancy among women at-risk for poor health outcomes. The Dad Project Goal: Empower fathers-to-be in order to help them become responsible and loving fathers. Abortion Changes You: Goal: To offer post-abortion women an intimate and safe place where they can talk, reflect and heal. Teen Pregnancy: Goal: To educate students about the importance of building satisfying and mature relationships free from sexually transmitted disease, unwanted pregnancy and relational heartache. Sexually Transmitted Diseases One-On-One Health Education: Goal: To reduce the incidence of sexually transmitted diseases among at-risk populations by helping them increase their knowledge about the risk of engaging in risky sexual behavior.
