Community outreach mission dedicated to providing quality, holistic, comprehensive foot care.
Prevent T2 | Your Change for a Better Lifestyle
Diabetes prevention lifestyle/behavior change program to help prevent or delay type 2 diabetes. This program is specifically designed for people who are risk for type 2 diabetes, have prediabetes but have not been diagnosed with diabetes.
CUBS- Diabetes Support Group
Education and group fellowship in an innovative approach to better understand the emotional, social, and viable aspects of diabetes.
Nutrition for Wellness
Initiative dedicated to promoting healthy opportunities in the areas of diabetes meal-planning and nutrition. This program aids portions of the population whose adequate food supply is limited by a lack of financial resources. (c)
Corporate Health and Wellness Program
Initiative dedicated to improving the health and well-being of employees in the corporate world; through health indoctrination and group activities that reinforce positive lifestyle changes as a result of better workforce morale, efficiency and healthcare cost reduction. (c)
Wellness Wednesday
In an innovative approach to supporting people affected by diabetes, Wellness Wednesday is a community collaborative partnership with the DeKalb County Board of Health, and the Live Healthy DeKalb Coalition.
Get in the Game: Men's Health Initiative
Analysis approach conveying and fostering prominent education, community outreach and research to eliminate and impede health disparities impacting minority men diagnosed with Type 2 Diabetes.

What is the organization aiming to accomplish?
Aiming to successfully decrease the rapid growth rate of diabetes through community outreach programs that positively impacting underserved and rural populations-communities with a ubiquity of chronic disease by successfully educating program participants/beneficiaries on how to make healthier life choices. Also aiming to operate remotely and travel to communities with a high population of diabetes, to provide essential outreach services to meet the needs of those uninsured. As well as raise awareness of the dangers of smoking and second hand smoke exposure, and eradicating the burden of diabetes in older adult community, and promoting prevention strategics in adolescents.
What are the organization's key strategies for making this happen?
Strategies include a mobile/community outreach clinic. Also, to successfully expand established health and wellness support group to help those affected by diabetes understand the emotional, practical and social aspects of diabetes. Offering group fellowship, emotional support and Resources, Diabetes Education. Youth initiative strategies: offer assembly for youth to strengthen, and enhance health and wellness knowledge by providing activities that promote nutrition education, physical fitness, and childhood-socialization.
What are the organization's capabilities for doing this?
Capabilities of meeting goals through a deputation in place to fulfill task. Ability to successfully meet goals and aim through collaborative community partnerships which are the mechanism for executing and achieve the
comprehensive strategies that strengthen communities. Goal achievement with the aid of volunteers and staffing ability to successfully fulfill mission in an effective manner.
What have they accomplished so far and what's next?
Accomplishments include: lessening the burden of foot complications/amputation through annual community sock drive for those homeless affected by diabetes- - provided community outreach support to older adult community and successfully educated 180 seniors on diabetes self-management and hospitalization prevention--annual community outreach event, Taste of Health Expo to promote the diversity of fighting diabetes by protecting overall health-- Nutrition for Wellness, provided free cooking class a nutrition education to eradicate the burden of food insecurity.
