The Supportive Care Services Foundation aims to educate, engage, and advocate for the disinvested population including older adults, low-income individuals and families within the community by providing accessibility to community resources and services aimed to improve the health and wellbeing of individuals in our community.. The Supportive Care Services Foundation stresses the importance of providing services with cultural awareness to the aging population and their loved ones and to eliminate the barriers that are caused by health and economic disparities.

Health Promotion and Community Education

Promoting health and wellness for seniors and their loved ones by providing educational workshops discussing important topics and available resources available. Some topics we've discussed in previous workshops were patient's rights and choices, mental health awareness, alternative medical care such as Home Health and Hospice, and available community resources.

Supportive Care Services Foundation Care Coordination Program is a comprehensive “advocate” service that collaborates with partner organizations to provide case management and benefit advocacy services for person-centered care solutions to improve quality and well being for individuals and their loved ones. Case Managers are available to help with risk assessment, health care needs, financial assistance, government benefits assistance such as SSI, SSDI, IHSS and more.

Assist seniors and their families with obtaining information to available resources and provide referral services to partnering agencies that may better serve client's needs. We provide linkage and referral to community resources on an as needed basis. The Supportive Care Services Foundation acknowledges that to improve the health and wellbeing of the disinvested members in our community, it must be a collaborative effort between partnering agencies to better serve the community.
