The Tribute of Life Foundation was established to provide funding for the extra support Hospice Home Care, Inc. and Spring Valley Hospice, LLC patients and their families may need during this complicated time. Hospice Home Care and Spring Valley Hospice help patients live each day with comfort and dignity, while retaining individuality and control over their lives. Through skilled care, counseling, support and education, Hospice Home Care and Spring Valley Hospice provide their patients with the best possible quality of life. The Tribute of Life Foundation also seeks to educate the communities in which we serve about End off Life choices, pay tribute to those lost, and provide support for those left behind.

No One Is Alone

There is a significant homeless population who lack the resources and relational support needed to receive appropriate end-of-life care. We provide a warm, safe bed and 24/7 care for individuals who have no one else and no way to repay.

The Tribute of Life Foundation distributes 20,000 Five Wishes booklets annually to encourage end-of-life planning. This is a free, easy to use guide that becomes a legal document when completed, ensuring the highest level of care available at the end-of-life.

We deliver 1,000 lbs of food each week to patients and their families who otherwise worry about their next meal. Whether it’s financial stress or physical disability, we see the struggle and take action. Hot and ready to eat meals for the elderly living alone, perhaps a box of groceries to a family, or a special meal to celebrate a birthday; over half of those we for benefit from the comfort food program.

Our current goals are fairly straight-forward:

1. Help hospice eligible patients elect such care earlier, so that their remaining days can be lived out in maximum comfort and dignity, while retaining individuality and control over their lives.

2. Implement our "No One Is Alone" initiative in order to care for hospice eligible patients who struggle at the end of their journey because they are alone, homeless & without anyone to care.

We will continue use the "Five Wishes" (see below) as an entry-level tool to educate our communities and a pursuing additional methods to educate the medical community and others about the myths and truths of hospice care.

We have initiated an employee giving program whose proceeds will be applied to the funding of two fulltime beds at Hospice Home Care's inpatient unit for homeless hospice patients identified through our No One Is Alone initiative and have also dedicated our yearend public fundraising efforts to this cause.

We are also networking with local healthcare providers and others serving the homeless to better identify hospice eligible patients.

While the Tribute of Life Foundation is relatively new, the organizations we support have more than 20 years of experience in serving their communities by providing skilled care, counseling, spiritual/bereavement support, education and DME services for those in need of hospice services. Hospice Home Care also operates Arkansas' largest freestanding inpatient hospice facility and has committed to make two of its 36 beds available for patients identified through our "No One Is Alone" initiative.

One of the tools used to educate our communities is an end of life planning workbook called "Five Wishes". In 2017, we were able to distribute roughly 8,000 copies of this tool and are projecting that this number will increase to 20,000 by the end of 2018.

Our current cash position allows the foundation to not only maintain, but expand our education efforts. As we formalize our development strategy for 2019, we are confident that we can raise the additional resources necessary to fund our other programs at the level we have targeted.

The Tribute of Life Foundation is still relatively new, but has managed to fund all of the "Five Wishes" workbooks distributed to date.

Having successfully recruited a fulltime Executive Director just prior to the 4th quarter of 2018, we are excited about tackling the goals identified above, increasing our visibility within the communities we serve, and plotting a course for even more positive impact far into the future.
