Charting impact

Four powerful questions that require reflection about what really matters - results.

Our goal is to provide services that will improve the health of the residents of our area - the New River Valley region of southwest Virginia. We will do this through providing a scope of service and a workforce that will evolve in a manner consistent with the needs of our region and our target population. We plan to remain an exemplary community health center in Virginia through the provision of quality health care services.

Our strategies across all our three sites and service lines are to:
1) increase the number of patients served;
2) make a positive impact on the health of patients;
3) identify and take action to reduce patient barriers to care;
4) identify and pursue opportunities for growth;
5) operate according to a sustainable business model;
6) proactively address areas of noncompliance, risk, privacy and security.

We meet our goals using both exemplary personnel and detailed processes and protocols. Because we have an electronic health record with excellent reporting capabilities, we are able to capture and review data at any time. Our quality clinical staff provide the best care to our patients. Our leadership team, who has oversight of our protocols, consists of a Risk Director, Safety Director and Compliance Officer who manage HIPAA compliance, Patient-Centered Medical Home compliance and all federal regulatory issues.

In the past six years, we have added a third clinic location, added tele-health services at all locations, and expanded our staff from 25 to 70. We have added new services including pharmaceutical assistance and substance abuse support. Due to great need, we have recently purchased property in Montgomery County to build a new facility within the next 3-5 years.
