- Individualized Day Services - Provides activities and community-based opportunities for personal skill development for adults. We support socialization, participation in membership-based community groups and associations, and forming and maintaining relationships with other community members. - Community Membership - joining social groups, Advocacy Groups, and senior groups of choice. - Career Exploration & Employment - Job seeking opportunities, GED classes, Computer classes, entrepreneur, soft skills development, and resume building, etc. - Community Contribution - Assisting people to actively support their neighborhoods, improving the community as a good place to live and work. - Retirement - (for people 61 and older) - Relationships & Natural Supports.

CHSSP Supportive Living ensures that people we support have access to opportunities for individual, social, recreational and faith activities using community resources. We use discovery methods from person-centered thinking tools to learn about each person's interests, beliefs, culture, preferences, and needs. We help people to plan recreational activities, trips, and vacations, seek gainful employment, maintain optimal health, and retain strong ties with their families and friends.

Day Habilitation Program - CHSSP Day Habilitation is a community-based program designed to help support people to have access to and a choice of participating in various activities. Activities include, cultural, educational, health/wellness, and other opportunities designed to develop and continuously improve skills for adult independence. Day Habilitation provides training and skills development in community, volunteering and communication.
