What we aim to solve
Affinity Health Center is seeking to ensure access to healthcare for all who need it in York County, SC and the surrounding areas. Affinity believes that patient-centered care, provided in a culturally competent manner should be accessible for all people, regardless of income or insurance status.
Our programs
What are the organization's current programs, how do they measure success, and who do the programs serve?
Medical Care
Affinity Health Center provides primary care to persons of all ages, as well as HIV and Hepatitis C treatment services. Medical care is provided at a discounted rate, based on income, for individuals and families with low income. No one is turned away due to inability to pay.
Support Services
Affinity Health Center provides a host of supportive services to help patients address barriers to care. Services include case management, medication access, transportation assistance, etc. Services are provided at no cost to the patient. Additional specialized supportive services are available for people living with HIV/AIDS.
HIV Prevention Services
Affinity Health Center provides free, walk-in, rapid HIV testing services as well as testing for Hepatitis C, Gonorrhea, and Chlamydia. We also provide community education programs about HIV/STD prevention. Free condoms are also available at Affinity's main site.
Mental Health Services
Affinity Health Center provides counseling services for Affinity patients and assists with referrals to psychiatric services.
Dental Care
Affinity Health Center provides dental care for Affinity patients.

What is the organization aiming to accomplish?
Affinity Health Center's goals are focused on enhancing access to care and quality of care for those in need in our community. Affinity's vision is for a healthy and vibrant community where all people have access to exceptional and comprehensive healthcare.
What are the organization's key strategies for making this happen?
1. Enhance access to healthcare services in York County, particularly for those who are uninsured or underinsured, including Hepatitis C treatment, comprehensive family planning services, Pre-Exposure Prophylaxis (PrEP), behavioral health, primary care, dental care and pediatric services.
2. Enhance quality of care by providing patient-centered care in a medical home environment.
3. Expand HIV services
4. Ensure organizational financial stability
5. Develop key community partnerships that position Affinity to better serve patients, including formalized relationships with educational and training programs in the region.
What are the organization's capabilities for doing this?
Affinity Health Center has 19 years of experience in providing health care for vulnerable and disadvantaged populations in the community. Affinity Health Center has strong communitiy partnerships and a strong commitment to collaboration. Affinity has sound fiscal management and practices that have resulted in being debt-free with an operating reserve. Affinity has committed and qualified staff who are led by a strong leadershp team and governed by a dedicated and responsible Board of Directors.
What have they accomplished so far and what's next?
Affinity Health Center has provided care to more than 6,600 individuals over the last year. 60% of these individuals are uninsured and 70% of them live at or below the federal poverty level. Affinity is vital resource in the community for providing affordable, high-quality care to persons in need. In 2018, Affinity relocated to a new facility allowing significant expansion of primary care and dental care. Affinity added an on-site pharmacy in 2018. In 2019, Affinity added integrated behavioral health services including treatment for opioid use disorder.
