What we aim to solve
Wesley Woods strives to create communities of connection, well-being and promise for older adults, regardless of income, by providing high-quality, affordable housing and services. These services include a healthcare component provided through our licensed assisted living and skilled nursing programs, a robust, outcomes based wellness program, and spiritual support through our on-site Chaplains.

What is the organization aiming to accomplish?
We envision a world in which older adults are celebrated for the lives they have lived, the wisdom they share and everything they have yet to teach us. We do that by creating communities of connection, wellbeing and promise for the older adults we serve and the team members who engage in fulfilling our mission.
What are the organization's key strategies for making this happen?
Safe, affordable housing that facilitates connection is a key social determinant of health. Our communities and our programs support and facilitate longevity and healthy outcomes. For example, falls are the leading cause of injury-related death among older adults 64 years and older. Our wellness program seeks to reduce the number of falls and the severity of injury.
Additionally, our on-site Service Coordinator is a resident advocate and service "connector" to insure the older adults we serve are connected to the services they need to age well.
What are the organization's capabilities for doing this?
Each of communities have a Life Enrichment Director, a Wellness Nurse, Chaplain and Service Coordinator who all work in concert to provide a robust on-site program of services in addition to linking to community resources to ensure residents have access to the programs and services which facilitate connection and encourage positive wellness outcomes.
