Case Management

Our case management program gives our residents the one-on-one support they need to maintain their housing stability, manage their mental health and addictions, and develop independent self-care. For our residents and many people who have been homeless or marginally housed and living with psychiatric disabilities or other special needs, affordable housing combined with supportive services has proved to be the link to stability and an enhanced quality of life. Our case management program is the key to providing these supportive services and leads to promising results. Without this program our residents would not be able to secure and maintain their own housing in the community.

Grow the number of services made available to the current clients and increase the number of available homes to serve more clients.

Choices, Inc. is implementing a 5-point Development Plan as well as working with a community partner to increase the number of available properties; serving more clients in turn.

By partnering with Home Partnership Inc. (HPI) we will be able to make even more housing available to the community.

The Board of Directors has hired a new Executive Director with 30-years non-profit leadership experience. The organization now has a full team committed to building the organization. A 5-Point Development Plan has been implemented to increase the organizations revenues in focused directions. Community Partnerships have been formed to expand the available housing to the homeless families in Louisville, KY. On the Direct Services side, we are recruiting a team of volunteers to work with our Case Manager to begin a new set of services to our clients in the form of Life Skills sessions, youth programming, and mentorship opportunities with community partners. Our overall goal is for families to become self-sufficient and get a fresh start in life.
