TeleCare
TeleCare utilizes community volunteers to call homebound, isolated older adults in need of a friendly voice and safety net. This program helps keep older adults connected to the community and to local support services.
Crime Victim Services
The purpose of ElderServe’s Crime Victim Services is to support older adults in recovering from victimization, to increase access to the criminal justice system, and to increase awareness of elder abuse as a crime by providing court transportation, court advocacy, support services, crisis inter-vention, short-term emergency housing, and community outreach.
Social Services
– ElderServe offers case management services to address a variety of individual and family needs, such as post-hospital planning, transitioning to assisted living, budgeting, living well, and other community service connections. The goal in providing these services is to allow the individual to age in place.
Senior Companion Program
The Senior Companion Program promotes volunteer opportunities for income-eligible seniors age 55 and older. These trained volunteers are matched with frail older adults in the community who need support and assistance with the tasks of daily living to maintain independence.
HomeCare
ElderServe HomeCare provides non-medical services for older adults or disabled persons who want to continue living in their own home but need assistance with their daily activities. Services include Personal Care, Home Management, Caregiver Relief, Concierge, Financial Helper, and assisted care devices.
Senior Center
Seniore Center provides activities and educational programs free of charge and a complimentary lunch.
