Community Networks Program
The Community Networks Program, a collaborative initiative with Meharry Medical College, is designed to reduce and eliminate cancer health disparities through community based participatory research, education and training. The CNP focuses on five specific types of cancers: Breast, Cervical, Colorectal, Lung and Prostate and targets African American men and women over 21 years of age.
Community Health Center Program
Primary ambulatory medical, OB/GYN and dental services regardless of an individual's insurance status or ability to pay. Ancillary services available including 340b Reduced Price Prescription Medication Program, Onsite Pharmacy, Podiatry, Ophthalmology, Radiology, Pediatric Medicine, Pediatric Dentistry, Adult Medicine, Adult Dentistry, Diagnostic Laboratory, Social Services, and outreach.
Healthcare for the Homeless
The Healthcare for the Homeless program is designed to provide primary and preventive healthcare services to homeless individuals in Memphis and Shelby County. The HCH program is open to any person determined to be homeless and in need of supportive services.
Ryan White HIV/AIDS Part A Program
The Ryan White HIV/AIDS program is designed to provide medical care, confidential counseling, community and social service resources to HIV/AIDS infected persons. The program is open to all uninsured persons who are infected with the HIV/AIDS virus and in need of primary care, case management and other supportive services.
W.I.C (Women, Infants & Children)
The W.I.C. program is designed to provide supplemental food and nutrition education that promotes good health for pregnant, breastfeeding and postpartum women, infants and children up to age five. The program is open to pregnant women, women who have a baby less than six months old, those of childbearing age, and children and infants who are at nutritional risk, live in Shelby County and meet liberal income guidelines. Participation requirements include proof of the following at each certification visit: income, identity of person receiving vouchers, residency, and shot record for infants and children.
The Blues Project
A 3-year program aimed at reducing health and social risk factors leading to infant mortality. In addition to assisting pregnant women to gain access to quality prenatal care, The Blues Project also offers pregnant and post-partum women additional levels of support through group-based health education, individualized case management, social support, as well as valuable assistance with navigating and utilizing the various community based resources and aid programs available. Services are provided to participating mothers from onset of prenatal care through the child's 2nd birthday.
340b Reduced Price Prescription Drug Program
Thanks to Memphis Health Centers' status as a federally qualified health center through the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA), our on-site pharmacy is able to offer our patients free and reduced price prescription medications under the 340b Drug Program. Section 340b of the Social Security Act implemented limitations on the cost of covered outpatient drugs to certain federal grantees. Participation in the Program results in significant savings estimated to be 20% to 50% on the cost of pharmaceuticals.

What is the organization aiming to accomplish?
Memphis Health Center provides primary, preventive and enabling services to the residents of Shelby & Fayette counties, Tennessee, with an emphasis on serving the medically under-served and special populations within its service area. MHC administers a wide range of primary and supportive care services, including Adult Medicine, Pediatric Medicine, Obstetrics/Gynecology, Podiatry, Ophthalmology, Adult & Pediatric Dentistry, 340b Discount Pharmacy, Radiology, Laboratory, Behavioral Health, and Social Services. In addition, the Center partners with key community resources to improve access and availability of assistance to its large patient population, such as the Women, Infant & Children (WIC) program, Meharry Medical College's Community Networks Program, The Tennessee Breast & Cervical Cancer Program, The Blues Project, Porter-Leath, A Step Ahead Foundation, the Ryan White Part A HIV/AIDS and Early Intervention Services Programs, the Shelby County Health Department's HIV Prevention Program, Blue Cross Blue Shield of Tennessee's Patient Centered Medical Home Program, Healthy Shelby, Catholic Charities, Harbor House, CAAPs, the Salvation Army shelter, Synergy Treatment Centers and many others. MHC's long-term goal is to increase patient access to care to exceed 65,000 annual patient-provider encounters and to demonstrate consistent improvement in the overall quality of its patient health outcomes by exceeding annual quality targets in 14 key health conditions, such as Diabetes, Hypertension, Asthma, Low Birth Weight Deliveries, Prenatal patients entering care within their 1st trimester of pregnancy, Mental Health Referral Compliance, Homeless Referrals for Mental Health & Substance Abuse Services, Tobacco Use Assessments, Tobacco Cessation Counseling, Adult Weight Screening & Follow-Up, Pediatric Weight Assessment & Counseling, Pediatric Oral Health Service Utilization, & PAP Testing Compliance.
What are the organization's key strategies for making this happen?
MHC's core strategies to achieve greater patient access and to improve quality health outcomes include the following:
1) Adopt/implement an electronic health record system and integrate a patient portal
2) Raise funds to renovate, expand and modernize the main site clinic on Crump Boulevard in anticipation of higher patient demand for service associated with future Medicaid expansion and health exchanges under the Affordable Health Care Act.
3) Raise funds to renovate and modernize the Rossville clinic site facility to serve as a better representation of the quality of care available through MHC.
What are the organization's capabilities for doing this?
What have they accomplished so far and what's next?
MHC began its conversion to an Electronic Health Record system in early 2013 in order to improve the availability of key health information and efficiency for its providers. Once complete, patients will be able to access their own health record information via a patient portal and will become much more involved in their own treatment and care programs. The electronic system is expected to eliminate the deficiencies of a paper record system, improve operational efficiencies throughout the patient experience, speed turn times for third party record and information requests, minimize time between patient encounters and claims submission, and provide real-time access to key quality health outcome performance across the Center's entire patient population. This transition is expected to be complete by the end of 2013.
The patient flow process at MHC's main site location is disjointed and inefficient as a result of expansion and add-ons to the facility as the demand for services has grown over the 40+ years since the building's original construction. In addition, much of the facility, its improvements and furniture and equipment are aging and in need of replacement. The announcement of MHC's federal capital award totaling $4.725m in 2012 by the US Dept. of Health & Human Services, Health Resources & Services Administration (HRSA) designated to build capacity, renovate, modernize and expand the Crump facility is a potentially organization changing development. MHC must raise nearly $500,000 of its own funds to contribute towards the project, but this obstacle has not slowed the Center's progress towards completing the project. A capital campaign to raise financial support and donations from the general public, private foundations, associations and area businesses is underway and the Center anticipates breaking ground in October, 2013 with a scheduled completion date in mid-2014.
In addition to the above project, HRSA also announced an award of $500,000 in immediate facility improvements funds to be used to renovate and eliminate lingering facility issues at MHC's Rossville Health Center clinic located in Rossville, TN. This facility was originally constructed in the early 1970's and had begun to experience problems common for buildings it age, such as roof issues, failing HVAC systems, and aging equipment. MHC must raise nearly $100,000 of its own funds to supplement the federal assistance provided under the Affordable Care Act. The Rossville project broke ground in early 2013 and a grand re-opening is planned for August 2013.
