Charting impact
Four powerful questions that require reflection about what really matters - results.
What is the organization aiming to accomplish?
To improve the health and well-being of the people and communities that we serve by ensuring access to compassionate, quality, and culturally-sensitive preventive and primary health care services.
To establish and coordinate prevention and educational activities available to the general population and particularly to groups experiencing disproportional health disparities.
To provide comprehensive healthcare services, regardless of their ability to pay, to uninsured and underinsured individuals experiencing limited access of care.
What are the organization's key strategies for making this happen?
Enhance Community Outreach & Engagement
Expand Community Access & Delivery
Strengthen Organization Culture
Achieve Operational Excellence
Increase Fiscal Sustainability
What are the organization's capabilities for doing this?
In 2019, Cempa Community Care expanded to become a Community Health Center to help all people fight all illnesses. Cempa provides care for low-income, uninsured, and underinsured patients. The Cempa Community Care Clinic (CCCC) now serves over 1,400 clients directly linking them to a pipeline of services. As champions for healthy communities Cempa is dedicated to providing care without question or judgement, once you’re here - we’re here for you.
Cempa has also been the first community health center to both offer and promote pre-exposure prophylaxis, commonly referred to as PrEP. This once-a-day pill is used in the fight to prevent the spread of HIV in local communities by providing protection for people who do not have HIV but who are at substantial risk of becoming infected. Clients who receive a positive HIV result are linked to a medical case manager to establish a treatment plan, provide continued follow-up, support services (such as housing, transportation, or food assistance), and referrals for mental health counseling or other necessities.
In addition to treatment of HIV, the CCCC provides treatment to those infected with Hepatitis C. Cempa is the only community-based organization that offers income assistance coverage for the costs of medication, labs, and office visits to qualified individuals. By partnering with national assistance programs, Cempa has been able to reduce the out-of-pocket costs for someone without insurance from an average of $100,000 to less than $600.
Cempa implemented an organization-wide Trauma-informed Care training to activate a systemic trauma-informed evolution of our programs, staff, leadership, procedures and policies, ensuring that all persons feel safe,
welcome, and affirmed. Through the integration of Trauma-Informed Care, Cempa has been able to build a strong organization culture and create operational excellence so that staff are equipped with the tools and knowledge they need to improve efficiencies and achieve quality outcomes by being able to better serve the needs of our
clients. Cempa is becoming a more affirming and welcoming organization for individuals who may feel cast off, ignored, or abused by society or other health organizations that may not understand their specific needs and past
experiences.
In July 2021, Cempa expanded to include a full-service, on-site pharmacy providing person-centered services that help bridge the gap between the pharmacist and health care providers. Over the past few years, the team at
Cempa has continuously sought out ways to broaden our service offerings so we can be a healthcare provider that meets as many needs as possible for our clients.
What have they accomplished so far and what's next?
Leading the way in care for people living with HIV/AIDS (PLWHA) for thirty-five years, Cempa Community Care, formerly Chattanooga CARES, has expanded to become a community health center that fights all illnesses. Today, we provide treatment, prevention, and support services to those impacted with health disparities, with a
focus on primary and infectious disease care. We offer a full range of services, including nutritional and mental health counseling, laboratory services, and preventative health maintenance, along with community outreach and prevention.

- We ask team members to identify racial disparities in their programs and / or portfolios.
- We analyze disaggregated data and root causes of race disparities that impact the organization's programs, portfolios, and the populations served.
- We disaggregate data to adjust programming goals to keep pace with changing needs of the communities we support.
- We employ non-traditional ways of gathering feedback on programs and trainings, which may include interviews, roundtables, and external reviews with/by community stakeholders.
- We disaggregate data by demographics, including race, in every policy and program measured.
- We have long-term strategic plans and measurable goals for creating a culture such that one’s race identity has no influence on how they fare within the organization.
- We have a promotion process that anticipates and mitigates implicit and explicit biases about people of color serving in leadership positions.
- We seek individuals from various race backgrounds for board and executive director/CEO positions within our organization.
- We have community representation at the board level, either on the board itself or through a community advisory board.
- We help senior leadership understand how to be inclusive leaders with learning approaches that emphasize reflection, iteration, and adaptability.
- We engage everyone, from the board to staff levels of the organization, in race equity work and ensure that individuals understand their roles in creating culture such that one’s race identity has no influence on how they fare within the organization.
