Our organization is focused on serving at risk areas with high incidences of HIV/AIDS, Hepatitis C and STD. In some areas, even though HIV/AIDS, Hepatitis C and STD are diagnosed, only 50% of patients receive treatment.

What are the organization's current programs, how do they measure success, and who do the programs serve?
HIV and STD Medical Services Program
CAN Community Health's medical services program is designed to provide a continuum of care and medical treatment services to individuals and families living with, and affected by, HIV/AIDS, Hepatitis C, sexually transmitted diseases and other diseases. Patients have encounters with physicians, case managers, nursing staff, and specialty medical providers who assess and design an interdisciplinary plan of care, regardless of their ability to pay.
Dental Services Program
The dental services program provided by CAN Community Health is designed to provide treatment and services to those living with and affected by HIV/AIDS, Hepatitis C and STDs, regardless of the ability to pay. Dental problems that are left unaddressed to a person with HIV and other infectious diseases may lead to a host of other medical issues. Accordingly, our dental program provides services such as cleanings, dental evaluations and exams, extractions, dentures, oral cancer screenings, x-rays, as well as several other services to meet the needs of the patients. These dental services are an important part of the success in the overall plan of care for an individual living with HIV. Education is reinforced at each dental visit as well as at periodic case management and medical visits.Sadly, patients living with HIV/AIDS are often refused services by dental practitioners. CAN Community Health is committed to providing this and all services to improve our patients' lives and health.
Prescription Assistance Program
A driving force in CAN Community Health’s mission is the removal of as many “barriers to care” for its patients as possible. Commonly, one of these barriers is the access to prescription drugs needed to treat and prevent the transmission of HIV/AIDS and other conditions. To accomplish this, CAN Community Health participates in the federal 340-B Drug Discount Program. The program is designed to help covered entities, like CAN Community Health, "stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services."

We have over 1,000 clients served in Sarasota, FL and over 9,000 statewide. A new clinic getting ready to open in Miami will increase those numbers dramatically.

What is the organization aiming to accomplish?
To provide a continuum of care to our current patients as well as participate in studies focused on the eradication of HIV, Hepatitis C, and STDs. Our education and community outreach team provides outreach education and mobile testing to 8+ Florida counties with ongoing plans to expand coverage.
What are the organization's key strategies for making this happen?
By participating in national study groups and utilizing our education and prevention division, we are participating in the battle to end of the HIV, Hepatitis C, and STDs.
What are the organization's capabilities for doing this?
Our education and prevention division serves the community with our mobile testing van, case managers for the newly diagnosed and peer navigators to guide patients through their journey. We also provide psychiatric and dental services to our patients in need.
What have they accomplished so far and what's next?
We currently serve over 10,000 patients with HIV, Hepatitis C and STDs.
