Charting impact

Four powerful questions that require reflection about what really matters - results.

The Foundation remains committed to working with hospice social workers nationally to help identify the patients and families they serve who lives have been changed not only by the health crisis they are facing but also by the financial crisis they face at the very same time by supporting them with financial grants to assist with everyday living expenses that are not met by the Medicare benefit.

To reach out to the public via social media efforts (web page, facebook, twitter) to align with like organizations to help educate the public at large about hospice and what it means, earlier access to hospice care, as well as the available resources to the patient population.

The Foundation For End-Of-Life Care is comprised of a well informed group of professionals who have made the hospice care sector their life's goal to bring about education, access, and quality care for those in need of hospice care. We are engaged in a broad stroke of social media outreach, engaging in local and national fundraising campaigns to also bring about awareness of hospice care.

The Foundation, since its inception has been helping families in financial crisis who are coping with the terminal illness of a loved one. More recenlty over the past six years the Foundation for End of Life Care, Inc., granted more than 6,000 special need grants for indigent patients and their families who are experiencing financial hardship. We provided essential items such as food, clothing, overdue rent and utility payments and last-minute travel for final reunions. Comfort items such as window air conditioners, pest control services and reading glasses also were funded for needy patients.
