The need for nonprofit organizations to serve Napa County’s senior population has never been greater than it is today. The population of senior residents in the county has increased by over 21% since the beginning of the millennium and is expected to continue to significantly increase over the next decade. Many of these community members have chronic diseases such as heart disease, diabetes, cancer, and Alzheimer’s disease. In addition to needing support to manage life-altering illness and end-of-life care, Napa County’s seniors are seeking services that will enable them to remain healthy and independent for as long as possible. Collabria Care aims to meet the needs of the growing Napa County senior population and their caregivers. We are committed to providing exceptional professional health care, expert resources, and compassionate support to those experiencing the transitions of aging, serious illness, or facing end of life.

Collabria Hospice

For more than 40 years, Collabria Hospice has cared for those facing the final stage of life. Our team of specialists are committed to meeting the physical, emotional, and spiritual needs of patients, caregivers, and families. Together we work to ensure that no one faces the end of life alone.

People with diseases and illnesses

The Collabria Day Program is the only Medi-Cal certified Adult Day Health Care program in Napa County. It provides state-of-the-art nursing care, rehabilitative therapies, social service support, social engagement and stimulation for adults suffering from serious illness and neurological disorders. The Day Program is also Napa County's designated Alzheimer's Day Care Resource Center providing specialized services for Alzheimer's patients, as well as education, support, and respite for their caregivers.

Individualized plans are created for each participant to maximize their independence, improve management of chronic conditions, enhance quality of life, and prevent hospitalization and/or premature nursing home placement. Underlying each care plan and all of our programming is our deep aspiration to create an environment in which each participant feels deeply respected, valued, and empowered.

The Collabria Palliative Care Program supports those with progressive or advanced illnesses, at any age and at any stage. We collaborate with patients’ existing doctors to enhance quality of life and provide additional support for them and their families wherever the patient calls home.
