What is the organization aiming to accomplish?
The Daughters began their ministry in healthcare in New Orleans at Charity Hospital in 1834; they continued management until the early part of this century. Hotel Dieu Hospital was founded and managed by the Daughters of Charity from 1859 until the end of 1992. Adhering to the ideals of service often leads to longevity and changes in the way services are delivered. The Daughters of Charity have exhibited this principle by caring for New Orleans for over 175 years. Incorporated in 1996 to perpetuate a Catholic health care presence in New Orleans, Daughters of Charity Services of New Orleans (DCSNO) offers holistic primary and preventive care services for pediatrics, adolescents, adults, and seniors at four clinics. Services include primary and preventive healthcare, pediatrics, dental, immunizations, chronic disease management, pre-natal care, behavioral health, health and wellness training, onsite laboratory, social services, health education group sessions, and onsite pharmaceutical services. Comprehensive, nationally recognized quality care is available to patients who are insured, underinsured, or uninsured.
DCSNO's most recognizable contribution to New Orleans is provided through its Marillac Model of Care (MMC). MMC enables patients to better manage their chronic illnesses. MMC allowed DCSNO to serve more than 25,000 patients and facilitate more than 70,000 visits last year. MMC is based on comprehensive, integrated services, utilizing an interdisciplinary team who can use their collective expertise to help improve patients' overall health and wellness. The comprehensive management team consists of a provider, medical assistant, registered nurse, behavioral health care managers, referral coordinator, and pharmacist, all of whom are primarily focused on population-based outcomes. A key goal of the MMC is to improve the care and management of chronic conditions such as asthma, diabetes, depression and high cholesterol.
What are the organization's key strategies for making this happen?
A key goal of the Marillac Model is to improve the care and management of chronic conditions such as asthma, diabetes, depression and high cholesterol. This goal is achieved by linking patients to a medical home and social wrap around services that helps to eliminate barriers to care, creates greater access to preventive care services, ultimately leading to better management of chronic conditions.
