What we aim to solve
Family Eldercare services are designed and developed to help seniors "age in place"-our fundamental goal. In the past 35 years, Family Eldercare has designed and developed an array of best practices programs to address needs of seniors and disabled adults for in-home care, caregiver support, legal guardianship, money management and service coordination in senior housing communities in Travis and Williamson Counties. We have expanded to offer home-based therapeutic counseling as well as classes using evidence-based curriculum in Chronic Disease Self-Management and Diabetes Self-Management. Additionally, Family Eldercare now provides service coordination to residents at the RBJ Center, four different HACA properties and four other nonprofit residential properties. Recently, we developed and now look to expand our Lifetime Connections Without Walls (LCWW), an innovative program that connects home-bound and disabled seniors with others via a telephone conference

What is the organization aiming to accomplish?
Vision: All seniors and adults with disabilities thrive with dignity in a supportive community
Mission: Supporting seniors and adults with disabilities in Central Texas to live with dignity in their communities by providing them and their caregivers with essential individualized services
Overarching Strategy: To focus on providing a continuum of quality care services to Seniors and Adults with disabilities in the low income category (200% or below) in Central Texas, centered on relationships and care

• To serve more people in a cost effective way while assuring sustainable funding sources

What are the organization's capabilities for doing this?
Board of Directors: A highly qualified, competent and diverse Board of Directors provides oversight of the agency.
Staff: Educated, trained and licensed staff. Each staff person receives an initial orientation from our HR Director, weekly supervision, annual evaluation that sets SMART goals.
Programs:
Service Coordination: provides case management to help seniors 'age in place' in nine senior housing communities including Housing Authority City of Austin public housing. Prevents 600 seniors from becoming homelessness or premature institutionalization in shelters/nursing homes.
Guardianship: provides legal guardianship protections to individuals who lack the mental capacity to make decisions or care for themselves. Protects and manages the assets of more than 400 clients.
In-Home Care & Caregiver Services: provides licensed personal care assistance and homemaker services to help older adults live as independently as possible. The program also offers training and support to family and informal caregivers. Provides 25,000 hours of caring help to 200 frail elderly and disabled adults who need assistance with daily living.
Money Management Services: provides case management, bill paying and representative payee services for individuals who are unable to manage their finances and are at risk of financial exploitation, homelessness and premature institutionalization. Assists 600 clients including veterans achieve financial stability.
Therapeutic Counseling: provides in-home counseling to help older adults and their families manage ongoing mental health issues, as well as issues as a result of the aging process. Brings peace of mind to 90 low/moderate income families and children.
Lifetime Connections Without Walls: provides educational, social and mental fitness activities to older adults via phone conference calls. Based on a nationally recognized model that enhances mental fitness and reduces depression among older adults. Helps 70 home-bound elderly and disabled adults stay mentally and socially engaged.
Lyons Gardens Residential Independent Living Community: 54 handsome units set on beautiful acreage in southeast Austin are home to both single and married seniors. Rent is on a sliding scale. Family Eldercare provides service connections and engaging activities to keep seniors feeling young at heart and sound of mind. Lyons Gardens is an award-winning model for safe, affordable, independent senior living. Provides 60 elderly residents with safe, secure, affordable apartments on a sliding scale.
Guardianship Estate Services: provides legal guardianship and protection for estates and trusts. Provides legal guardianship to 100 frail elderly persons who might otherwise be victims of financial exploitation.
Summer Fan Drive: distributes fans to low-income seniors, people with disabilities, and families with young children in 11 central Texas counties. Distributes more than 6,000 fans to 3,000 low income families.
What have they accomplished so far and what's next?
Family Eldercare was founded in 1982 by professionals concerned with supporting the family's role as caregiver for seniors. In 1986, the agency expanded to provide services to prevent abuse, neglect and exploitation of seniors and adults with disabilities. The agency later began providing in-home care and respite services for seniors and their caregivers. In 2009, Family Eldercare initiated a program that uses telephone conference calls to reach older adults who are primarily home-bound. Geriatric consultation and therapeutic counseling for seniors and their families was initiated in 2010. Family Eldercare has experienced dynamic growth over the past several years and currently serves more than 6,000 individuals annually in Travis County and Williamson County among others.
