As of 2017, Fort Bend County's population is 765,000 and the County reflects a diverse population with 33.1% White, 24.5% Hispanic, 20.5% Asian and 20.3% Black. The Asian-American population nearly quadrupled in size between 2000 and 2017, and the Hispanic and Black populations more than doubled. The immigrant population continues to grow in the County. (ref. source: www.understandinghouston.org) Fort Bend Care Center seeks to improve community wellness and address these key community needs: 1) 24% of people in Fort Bend County having limited English proficiency 2) 8.2% of the population in Fort Bend County living in poverty 3) 30% of households in Fort Bend County struggle to afford basic necessities 4) 44% of the census blocks in Fort Bend County are in the lowest quartile for Health Literacy Levels Fort Bend Care Center seeks to improve Fort Bend County's Wellness through Literacy, Health, spiritual and Wellness Programs

Fort Bend Care Center seeks to improve Fort Bend County's Wellness through Literacy, Health, spiritual and Wellness Programs.

Literacy
To improve community wellness through literacy, we provide English as a Second Language Class for all English Level 1 - 5. on Mondays and Wednesdays. To accommodate our neighbors' busy work schedule, we also offer free English Learning Phone App for students to learn English outside classroom setting. Besides English classes, we also provide weekly GED preparation Class. We also have weekly Citizenship Class for residents to learn about American history, civics, and English to prepare for their citizenship interview. In 2022, we seek to provide more support to connect students to job skill trainings.

Food
Every month we host a monthly food fair serving about 150 families and distributing about 12,000 pounds of fresh fruits, vegetables and healthy food to our neighbors each month. We offer adult and children programs while the neighbors are waiting for the food sorting.

Health
We partner with a Federal Qualified Health Center, AccessHealth and provide facility for the mobile clinic to offer medical service for our community, Monday to Friday. AccessHealth serves clients with or without health insurance.

Once a year in the fall, we host health fair to offer free flu shots, health screenings and health education programs to our community.

Lasting Transformation
We believe that lasting impact comes through authentic relationships. Therefore we seek opportunities to listen to our neighbors and develop relationships anchored in love, respect and dignity. We design our services based on their input, concerns and aspirations.

Neighbor Participation & Leadership
We believe that our neighbors can build upon their strengths to enrich their own lives and neighborhoods. Our goal is to enable neighbors’ participation and grow their leadership within the community.

Community Collaboration
We believe in community strengths and leveraging existing resources and assets. We strive to collaborate with community networks, local government, churches, non-profit organizations, community leaders and neighbors to address community needs and promote partnership in community development.

The Services We Offer
Education Classes- We offer ESL classes for non-native English speakers to learn English and acquire English proficiency life skills
GED Class- We offer GED preparation class to help students prepare for the high school equivalence testing. This will improve their employment opportunities.
Health Clinic- we partner with AccessHealth offering mobile health clinic in our facility. This allows the access to healthcare services for people with or without insurance.

More than 90% of the Care Center's work is accomplished by volunteers. In 2021, we tracked 4,137 volunteer service hours for our various programs

We establish collaborative partnership with government agency and local nonprofit organizations. In partnership with AccessHealth, we offer medical clinic service in our facility. In partnership with Second Mile, we are able to provide fresh produce for our local community who need healthy food.

We have effective board and staff leadership in program development and fund raising efforts.

The number of our ESL class registrants and food fair clients has quadrupled since 2018.

In 2021, we recorded a total of 2,210 ESL student attendance hours, 10,339 clients served at food fair, 175,562 pounds of food distributed. We also tracked 4,137 volunteer hours for various programs provided.

In partnership with AccessHealth, the mobile clinic is currently operating with about 200 patient visits every month. The clinic is expanding with the construction of a new clinic module. With the new facility and expanded services, we seek to serve more people who are without insurance or under-insured.

In our partnership with Houston Health Foundation, AccessHealth and other organizations, the 2022 See-to-Succeed Program provided free vision and eye glasses service to students in our community. The 2022 event has served about 450 students.

We hope to continue to strengthen our collaborative partnership in the community to expand our services. In 2022, we have launched East Fort Bend Children Program and summer camp. We are also in the process of developing effective ESL distance learning program to meet the enormous challenge in face of the pandemic.
