What are the organization's current programs, how do they measure success, and who do the programs serve?
Early Intervention
This program serves families and their children ages birth to three who have developmental delays, and begins with our agency coordinating an evaluation to determine the child's developmental needs and eligibility for services. Early Childhood Specialists and therapists (OT, PT, SP) then work with parents and their child in accordance with the outcomes identified in the Individual Family Service Plan (IFSP) to develop routines-based activities and learning techniques which can be used to enhance and promote the child's development. Our Early Intervention Service Coordinators facilitate the process of developing the IFSP. These services are provided in the family home and/or in community settings, and as a result of the COVID-19 pandemic, these services can also now be provided virtually. Many providers and families have found this to be an efficient and effective way to provide quality services.
Children's Extensive Support Program
This program provides specific targeted and cost-effective services and supports to assist a child who has intellectual/developmental disabilities and extensive medical and/or behavioral issues to remain in the family home. Children are eligible from birth to age 18. These services support the long-term stability of the family and prevent costly and disruptive out-of-home placement. CES services and supports supplement natural supports and generic community resources.
Supported Living Services
This program offers a wide menu of services and supports for persons with intellectual/developmental disabilities over the age of 18 who live with their family, or are responsible for their own living arrangement. These services can include employment or day program services, supported community connections, supported employment, mentoring, behavioral services, and others. We currently serve over 100 adults in this program.
Adult Comprehensive Services
This program provides a comprehensive array of services for adults with intellectual/developmental disabilities over the age of 18, many of whom require 24 hour support. Residential services include group homes (licensed by the Health Dept. for between 4-8 residents); Personal Care Alternatives (staffed home or apartment settings for 1-3 people); Host Homes (family homes operating as independent contractors serving from1-3 people), and Family Care Giver arrangements where parents/family members can receive reimbursement for caring for their son/daughter in the family home. This program has been especially helpful for parents who have had to quit their jobs to stay home and care for a loved one. Day Program Services include Supported Employment, Pre-Vocational Services, Day Habilitation Services, community-based day activities. Medical support and transportation services are also included in these Comprehensive Services as needed. We currently serve over 100 adults in this program.
