FSL provides home and community-based services and develops energy-efficient, affordable housing to promote health, independence and dignity for all. FSL serves the entire state of Arizona in regard to our affordable housing programs, and Maricopa County for our home and community-based services. All or our services are intended to assist people who want to age in place.

Home Health

We provide in-home nursing services for people living at home with a ventilator or trach.

ReCreacion, FSL's state-licensed adult day health services provides meaningful activities, socialization and assistance for adults during the day. At our three centers, we provide quality care and a supportive atmosphere for all of our members. You’ll be able to spend time with familiar faces in a safe and secure environment.

Adults of all ages, with a variety of cognitive and physical disabilities, are welcome and receive personalized services that meet their needs: medical, social and recreational. Our programs enhance members’ levels of independence while advancing positive well-being and health. Because of the variety of services that we offer, the adults experience a range of choices during the day.

FSL operates food pantries and two, professional kitchens that prepare and serve congregate and home-delivered meals to low-income seniors and adults with disabilities.

FSL also provides emergency assistance and case management to help low-income individuals and families out of a crisis and to avoid homelessness. Services include food boxes, rental assistance/deposits, utility bills/assistance, etc.

Adult foster care enables individuals to live in a home setting when they can no longer live on their own. Residents get assistance with daily living, including health care related services. Each resident is considered a participating family member in the home dynamic and benefits from the community feel, while still being able to maintain a level of independence.

The Maricopa County Adult Foster Care Home program provides a nurturing residential environment. Our program equips the foster care home with trained certified caregivers, sponsors and owners, each of whom can provide health related assistance. We also use outside service agencies such as hospice care when needed. Our goal is to offer a compatible licensed adult foster care home to each individual who can benefit from family care, and to help our residents retain their independence for as long as possible.

At FSL our goal is to provide safe, affordable and energy-efficient housing for families and individuals of all ages. FSL offers single-family homes for sale and affordable apartments for rent.

The assisted group living program through FSL operates homes throughout the Phoenix Metropolitan area. Our program has received wide recognition as a model of exceptional community care. All of our residential behavioral health facilities are state-licensed, and each home has a team of trained professionals who offer behavioral health and medical services to their residents. Additionally, all staff are certified caregivers who are trained in delivering behavioral health and personal care needs
