What we aim to solve
For individuals with developmental disabilities, local resources can be limited and without extra support, parents could be forced to move the family south or put their loved ones into institutional care. There are families who have very young children (ages birth to three) who may be at risk for developmental delays. Many families need or want guidance on how to build stronger relationships with their children, so that their children develop good social-emotional skills and grow up healthier. There are elders in our community who need additional assistance in order to remain in their own homes, so they don't have to move down south.

Early Learning Program
The Early Learning Program of Community Connections is a home-based, family-centered early intervention program for families and caregivers of young children ages birth to three. Services are provided in partnership with biological parents, foster parents, grandparents and any other important people in the child’s life. The Early Learning Program provides services for families with infants and toddlers with developmental delays and/or disabilities. The program also serves children who may be at risk for developmental delays including those born prematurely, those exposed to substances during pregnancy, or children who have experienced varying degrees of trauma.
Children's Mental Health
Children’s Mental Health offers children, youth and their families an opportunity to work with trained professionals to address life’s challenges. Services are selected based upon the needs and strengths identified by the parents, child, teachers, and our professionals during initial meetings. Services are intended to support the development of new skills for the enrolled child and their family and enhance the functioning of an entire family in their every day environments.
Senior & Disability Services
Assisting people with developmental disabilities, adults with physical disabilities, and seniors to achieve their life goals and remain in the community of their choice. All of our services are individualized and designed by the customer, family and our service coordinators with the goal of meeting specific individual needs. Services may include residential support, community-based services, supported employment, personal care services, chore and respite services, and veterans services.
