What is the organization aiming to accomplish?
PCL’s organizational vision is to create and continually evolve as an agency that is the most person centered provider of services to individuals with developmental disabilities. Creating the best training and consultation on issues related to developmental disabilities. It is our goal to be the preferred provider of these services and the quality standard by which others are measured.
Right to Receive Supports Needed
Each transition plan is unique and designed to provide a home that is supportive and a safe place to live. Support opportunities include a thorough needs assessment and planning meetings to ensure thoughtful choice of support.
Safe and Affordable Housing
People we support live in their own homes, in the town they want to live in and with people they like and choose to live with, or on their own if they prefer.
Community Participation
Tapestry services provide supports for people to be a valued member of their communities where they are appreciated for their contributions, skills and talents. Tapestry Services provide opportunities that encourage personal growth and creativity. Services include the following: Identifying a person’s interests through Person-Centered Planning and the ISP process; Assisting a person to find opportunities to enhance these interests by building relationships with others with similar interests; Assisting a person to research, schedule, and participate in these opportunities; Managing documentation and state billing requirements.
True Employment
Job Launch is a team of dedicated and certified employment professionals who believe everyone who wants to work, can work and there is a job for everyone. Our aim is to be creative and tenacious in providing support and that not working should be the exception. People will be hired because of their ability not because they have a disability. People are healthier, safer, and happiest with meaningful work.
Families
Families are a very important part of planning and providing the very best support given to their loved ones. We keep families in the loop with events, meetings, and surveys.
Active Lives
Whether it’s a night out at the movies or a concert, a quick trip to the beach or other local attractions, or a well planned vacation, people we support have very full lives of fun, adventure and experiences.
Self Advocacy
SPARC is self advocacy group that is made up of people with developmental disabilities who share a desire to change the way people with disabilities are seen by society. It is comprised of, and governed by, self advocates who make all the decisions that govern the activities, finances, time and education of its members.
What are the organization's key strategies for making this happen?
PCL offers a large variety of services for people with developmental disabilities. Many people we support even receive multiple types of services and we cater to the specific needs and the Individualized Support Plan (ISP) of each person. An ISP is a written plan that details the support needs, activities and resources that are important to each person’s unique preferences. A team that includes, and is often led by, that person meets often or as needed to determine goals for that person and then puts a plan in place on how to meet those goals.
Person Centered Services - People that we support are receiving services that are designed to their individual preferences and needs. Through the Person Centered Philosophy, we are always evolving and seeking to improve the services we provide, the language we use and they way people with developmental disabilities are represented.
24-Hour Care - Horizons is for people living in their own home that fits their preferences and lifestyle, and often with a roommate or a couple roommates who have similar preferences and make a good match.
Supported Living (SLP) - While PCL always encourages people to live as independently as possible, those we support in SLP are those who have actively expressed wanting to live alone, learn how to balance their own budgets, cook their own food and etc.
Individualized Services - The needs for people in this program could be having a Direct Support Professional (DSP) visit them a few times a week or even just as needed to assist with things like cooking meals, paying bills, grocery shopping, attending events, etc.
Job Launch - Job Launch is a community-based employment program that helps people with developmental disabilities find meaningful jobs and career opportunities.
Tapestry - Tapestry is an evolved, completely community-based, Day Services program for people who want to explore life outside their home on their own terms.
Roots Project - The Roots Project provides treatment based supports to youth and young adults facing the challenges of mental health and intellectual disabilities. Sometimes kids need to be supported outside the family homes for a variety or reasons. When that is the case, we are able to help by providing nurturing homes and staff dedicated to helping learn skills, build relationships, and return home when they are able. Fostering a close relationship with the entire family is a big part of those supports.
Cognitive and Behavioral Health Services (CBHS) - Developing quality relationships and help determine the needs for mental, physical and behavioral health. The CBHS team helps train our Direct Support Professionals (DSPs) that will be working with people and then continue to ensure good matches and continuing support need changes.
Health Services - Registered Nurses, Physical Therapist and Health Services Coordinator, ensure people supported receive the health care they need.
What are the organization's capabilities for doing this?
PCL's program framework will effectively deliver desired outcomes for the people we support and our team members.
PCL will grow and maintain a strong network of stakeholders including the people we support, family members, staff, local businesses, government officials, community members, advocacy groups and affiliate organizations.
PCL will continue to be a leader in advocating for people with intellectual and developmental disabilities through: following it's strong mission and values; engaging in state and federal advocacy; advancing relationships with PCL advocacy networks such as Oregon Resource Association (ORA), Employment First, Oregon Council on Developmental Disabilities, Oregon DHS, Oregon VR and Disability Rights Oregon.
PCL will continue to advance the necessary resources, systems, and supports to be a data-driven and performance oriented organization. The PCL Board Plan 2018-2021 outlines the four Key Result Areas and has strong measures in place to review progress and allow for flexibility when faced with challenges.
What have they accomplished so far and what's next?
PCL is proud of the many accomplishments and humbled and challenged by the fact that there is still so much to do. Breaking through barriers, addressing challenges and moving forward is a fine art. PCL is thankful to have a team of skilled professionals that make this happen every day.. At PCL we are ever more committed to our mission and the people we support. Here are a few highlights from the most recent Board Plan Review:
Key Result Area 1: PCL is a Great Provider of Person Centered Services
Embedded the philosophy of Person Centered Language and Philosophy into all PCL processes including orientation, onboarding, values playbook, trainings and organization materials.
Key Result Area 2: PCL is a Great Place to Work
Ranked in the 2018 top 100 best nonprofits to work for in Oregon. Developed innovative and creative recruiting opportunities and focus on retention of our qualified, long-term members. Percentage of interviewed applicants increased 34.5%. Hired 84.2% of applicants interviewed.
Key Result Area 3: PCL is a Great Partner with our Families, Communities & Stakeholders
Developed and maintained an ever-growing group of employment partners. Regularly host Open Door Tour connecting with educating new businesses and partners. PCL was awarded the Monmouth / Independence Best Business of the Year.
Key Result Area 4: PCL is a Great Steward of the Resources Received to Provide Services
Continue stewardship of resources with completion of tracking system to better capture expenditures in greater detail. Also, began a huge endeavor of researching and analyzing funding structures to understand how PCL can address wage issues within the current funding structure.
Looking towards the future PCL will continue with the persistence and dedication to our purpose of being a great provider of person centered services, a great place to work, a great partner and steward of PCL resources. As outlined in our 2018-2021 Board Plan we will:
