Providing nutrition, non-emergency transportation and case management services to seniors who do not have access to or cannot use personal or public transportation services.

Meals on Wheels
Meals on Wheels: Menus are designed by a Registered Dietitian. Each delivery consists of a hot, ready-to-eat meal, a cold meal to eat later in the day, and a breakfast meal for the following morning. Volunteers deliver meals Monday through Friday between the hours of 10:30 AM and 1:00 PM. Frozen meals are available for weekends.
Congregate Meals
Congregate Meals: to complement Meals on Wheels and to facilitate socialization, Age Well Senior Services serves hot, nutritional lunches for active older adults Monday through Friday at 10 senior centers in south Orange County. Seniors enjoy over 100,000 lunches each year, while dining with their friends at senior centers.
Case Management and In-Home Support Services
Case Management and In-Home Support Services: Seniors are often faced with chronic or catastrophic circumstances that stress their coping skills, personal/financial resources, and learning and functional abilities. This can be a daunting experience for elderly persons and their families. Case Management is the thread that weaves through the core programs enabling seniors/frail population to remain independent within their community. Case Management is a collaborative process that assesses, develops and implements care plans, coordinates, monitors, and evaluates options to meet a person's needs through available resources. Age Well Senior Services' case managers help at-risk individuals understand the varied community services that are available to them, and then guide them through the steps required to obtain those services.
Transportation Program
Transportation Program: Age Well Senior Services' Non-Emergency Medical Transportation Program provides seniors (60 years of age and older) with transportation to their non-emergency medical appointments including therapy, dentistry, hearing, dialysis, prescription pick-ups, testing and x-rays.

Our vital services are specially designed to help enable seniors to stay in their own homes and maintain quality of life. They emphasize a continuum of care that most cities are unable to provide for their aging population. Our network of services provides a spectrum of support based on level of need — whether you are an active senior looking for daily activities or a homebound senior in need of daily meals, we are with you every step of the way. Our goal is to provide hope and independence for an ever-expanding population of seniors and enhance the quality of life for all in the communities we serve.
