What are the organization's current programs, how do they measure success, and who do the programs serve?
Adult Developmental Program
Adult Development Program (ADP)
The Adult Development Program (ADP) is a licensed adult day program for individuals with moderate to profound developmental disabilities located in Lafayette, CA. ADP focuses on teaching and coaching individuals in the areas of activities of daily living, self-advocacy, and community integration. ADP also specializes in providing necessary supports for people with behavioral concerns and physical conditions. Such supports include teaching replacement behaviors for behaviors that interfere with the individual’s success, assistance with personal hygiene, feeding, toileting, and toileting hygiene, improving communication skills, and other necessary supports. Skills are taught both on-site in actual and simulated settings, and individuals are provided ample opportunities for practicing newly-learned skills in real-life community settings.
Adult Residential Model
Las Trampas provides residential facilities that are licensed through the California Department of Social Services-Community Care Licensing Division. These residential facilities offer a home to individuals with 24-hour staff support. Currently, Las Trampas operates two specialized residential homes in Contra Costa County – both of which are designed to support individuals with behavioral concerns. The emphasis is to provide necessary supports to maintain community placement in a home setting. Such supports and training include, but are not limited to: Self-advocacy, community integration, activities of daily living, behavioral support and teaching socially appropriate replacement skills, and medical services coordinated by board certified registered nurses.
Supported Living Services
As one of the original pilot programs funded through Community Supported Living Arrangements, Las Trampas has provided Supported Living Services since 1990. Each individual has a personal Facilitator who works with the individual and staff to ensure that the five core beliefs--a home of one’s own, choice and self-direction, developing relationships, community membership and participation, and flexible and tailored services--are supported to ensure the individual’s success in community living. One of the most important aspects of the program is helping individuals to develop a circle of support consisting of friends, relatives, neighbors, community members, and others to help them to reach their personal goals and ensure personal choices are respected.
Independent Living Services
The emphasis in this program is to provide necessary training to increase independent living skills with the goal of decreasing reliance on structured supports. Rather than completing tasks for individuals, they are provided with the tools and training to complete these tasks and responsibilities independently. Core training areas include, but are not limited to: money management; cooking, meal preparation and menu planning; nutrition; grocery and personal training; personal health, including accessing medical services and medication management; mobility and transportation training; self-advocacy; household management and cleaning; accessing community resources; developing independent social recreation opportunities; and other training as required to increase independent living skills.
