Community Health Services
LifeLong Medical Care is an innovative, nonprofit, Federally Qualified Health Center serving in Alameda and Contra Costa counties and community for almost 50 years. LifeLong has over a dozen primary care health centers, four dental centers, two dental vans, three school-based health centers, a supportive housing program, mental health services, and immediate/urgent care centers.
LifeLong believes everyone should have access to high-quality health and dental care, regardless of the ability to pay or immigration status. LifeLong accepts Medi-Cal, Denti-Cal, Medicare, most managed care plans and most insurance plans. If you are not insured, LifeLong offers uninsured programs and a sliding scale payment option, based upon family size, and income.
