What we aim to solve
Everyone deserves end-of-life care that is aligned with their beliefs, wishes, and values; and each patient should have access to the physical, emotional, and spiritual support they need to spend their last days with dignity.
Our programs
What are the organization's current programs, how do they measure success, and who do the programs serve?
Hospice
Tailored to individual needs and wishes, hospice provides compassionate comfort care to people with an estimated life expectancy of six months or less. Our interdisciplinary hospice teams provide medical care, symptom management, and emotional and spiritual support for patients and their loved ones.
Transitions
To help individuals and families maintain continuity of care, our transitional care offerings include social work, nurse consultation, and volunteer assistance before and between our other programs.
Hospice House
Our hospice house offers a peaceful, homelike option for people whose end-of-life needs cannot be met at home or in a hospital setting. Individuals typically spend a few days to a few weeks in the house, receiving professional, 24-hour hospice care and family support.
Community Education
We help people gain peace of mind about living and dying well through open dialogue and educational events. Our local partnerships and programs help individuals and clinicians become better informed, start conversations, and create a more compassionate community.
Grief Support
For those who are grieving or facing the death of a loved one, we offer individual counseling, free community support groups, and other special programs, including grief support for children.
Palliative Care
Palliative care focuses on symptom management and quality of life for patients with advanced chronic illness who are not ready for hospice.

What is the organization aiming to accomplish?
Mission Hospice & Home Care honors and supports people's wishes for the last phase of life by providing our community with exceptional end-of-life care and education. We envision a community in which death and dying are recognized as a natural part of life, and everyone is treated with compassion and respect.
What are the organization's key strategies for making this happen?
Mission Hospice & Home Care develops an annual strategic plan focused on four main areas: personalized, quality care; culture and leadership; strategic and financial sustainability; and innovation for our community.
What are the organization's capabilities for doing this?
In addition to our professional staff, we have a dedicated group of more than 250 volunteers who give thousands of hours each year to our patients and families. We are led by a committed Board of Directors who have deep roots in the community, and we received more than $2M in donations and grants in 2018 to support our care.
What have they accomplished so far and what's next?
In the past 10 years, Mission Hospice & Home Care has grown from serving 25 patients and families a day to hundreds of patients and families a day. With the leadership of our Board of Directors, we have expanded our care offerings, opened San Mateo County's first hospice house, and enhanced our community education and bereavement programs.
In the coming year, we expect continued growth, and also new challenges. The healthcare landscape is changing significantly -- in 2010, 49% of hospices were nonprofit; in 2017, that number dropped to 25%. With increased for-profit competition and the rising costs of delivering care, it will be more important than ever to partner with like-minded organizations to preserve non-profit, community-based and mission-driven hospice and palliative care in the Bay Area.
