Casa Colina Hospital
Freestanding 68 bed acute inpatient hospital specializing in rehabilitation to the CMS Inpatient Rehabilitation Facility (IRF) standard that provides 21,778 patient days of services a year. The Hospital also includes Outpatient Rehabilitation Services delivering 55,000 treatment visits a year, 19 Physician Clinics with 27 specialist physicians, a satellite Outpatient Rehabilitation service in Azusa, and a pediatric outpatient rehabilitation service with specialty programs in autism and early intervention for developmental delay that provides 13,100 treatment visits a year.
Transitional Living Center
The Transitional Living Center provides a residential environment for individuals with brain injury and spinal cord injury where they can learn the cognitive, behavioral and social skills to prepare them to re-integrate into home and community living. It is frequently used after a course of inpatient hospital rehabilitation for individuals who face challenges in the areas of daily living such as organizing and carrying out tasks during the day, managing behavior and emotions, and interacting effectively with family and friends.
