What we aim to solve

SOURCE: Self-reported by organization

As a premier integrated delivery system we provide high-value healthcare to the communities we serve, managing the population's healthcare needs, while also creating a model of care that thrives in California's changing healthcare arena. In response to community need, we serve a greater population throughout the continuum of care. We develop services central to our mission and convenient to our patients. We partner with other non-profit organizations, where appropriate, to develop a comprehensive network of health services through a seamless delivery system. We are the preferred, non-profit physician and outpatient provider of services to the communities we serve.

Charting impact

Four powerful questions that require reflection about what really matters - results.

Our mission is to provide an excellent healthcare experience, recognizing our priority is the patients we serve. We do this using a patient-centered approach to medicine. We build upon strengths in population management, providing our patients with superior outcomes through best practices in medicine. Exploring new, different and better ways of delivering expert medical care with compassion is central to our mission of serving the needs of our community.

Our strategies to achieve our mission are to deliver an excellent healthcare experience, to promote a culture of accountability, to create a sustainable mission and to meet the community need. The focus of our strategy is our people, facilities and process. We invest in personnel who recognize our patients are the focal point of our mission. We develop facilities that promote well-being and are conveniently located to serve our community. We adopt information technology to measure and improve the quality of care. We focus on providing an efficient delivery of care through continuous process improvement.

Sansum Clinic invests in the brightest, most talented physicians as they and our dedicated personnel are our future. We partner with our doctors to advance our mission and vision. We ensure our success through talented leaders who are empowered to set the direction of the organization. We ensure the public needs are well served through a strong community Board of Trustees. We partner with payers, providers, and purchasers that share our common goal. We deliver affordable, value-based care by reducing administrative and care costs.

Sansum Clinic expanded the services available to patients in our communities through initiatives such as expanding our urgent care, constructing a new cancer center, implementing a Patient-Centered Medical Home and broadening our electronic health records capabilities. Sansum Clinic will continue to invest in population health with the goals of improving the quality of healthcare provided in our community and lowering the overall cost of healthcare.

Our biggest challenge right now is vaccinating our patients and the community for COVID-19. There's a lack of vaccinations at this time and our hope is that more are coming soon.
